Major depressive or bipolar disorder with psychotic features
psychotic symptoms occur exclusively during mood episodes.
schizophrenia
mood symptoms, if present, are brief and not prominent.
schizoaffective disorder
major depressive or manic episode occurs concurrent with symptoms of schizophrenia.
lifetime history of delusions or hallucinations for 2 or more weeks outside of mood episode is present.
mood episodes are prominent and recur throughout illness.
any patient having depression
should be screened for a history of mania in order to differentiate unipolar from bipolar disorders.
major depressive episode and history of a manic episode
Bipolar 1 disorder ( pt can have delusions, hallucinations when they experience manic and depressive episodes).
depressive symptoms include?
depressed mood, hopelessness, hypersomnia, low energy/lethargy, impaired concentration, and psychomotor retardation on mental status examination.
manic episode with psychotic features?
psychotic episode 3 years ago, grandiose delusions, decreased need for sleep, and impulsive spending.
absence of psychotic symptoms outside of mood episodes
rules out primary psychotic disorder.
