Bipolar I DSM criteria (there are a lot….) Part One
Manic Episode:
- elevated, expansive or irritable mood and abnormally and persistently increased goal-directed activity or energy lasting AT LEAST ONE WEEK
- During this period three or more of the following symptoms are present to a significant degree and are noticeable: inflated self-esteem or grandiosity; decreased ned for sleep; more talkative; flight of ideas; distractibility; goal-directed activity; involvement in activities that have a high potential for painful consequences
- the mood disturbance is sufficiently severe to cause marked impairment in social or occupational functioning or to necessitate hospitalization
- not attributed to drugs (this is how someone on meth acts fyi) or medical issues
Bipolar I DSM Criteria part two
Hypomanic Episode
- period of abnormally and persistent elevated, expansive or irritate mood and abnormally and persistent increased activity or energy lasting AT LEAST 4 DAYS IN A ROW
- During the period of disturbance three or more of the following need to occur: same as mania
- episode is associated with an unequivocal change in functioning
- disturbance is observable by others
- not severe enough to cause impairment in life
- Not due to drugs or medical issues
Bipolar I DSM part three
Major Depressive Episode
- Five or more symptoms must be present for the same 2 week period and at least one needs to be either depressed mood or loss of interest or pleasure: insomnia or hypersomnia; psychomotor agitation; fatigue or loss of energy; feeling son worthlessness or excessive or inappropriate guilt; diminished ability to think or concentrate; thoughts of death, suicidal ideation w/ or w/out plan and attempts
- causes clinically distress
- not attributed to meds or substances
Bipolar I Disorder DSM Part four
Criteria have been met for at least one manic episode
Occurrence of the manic or major depressive episodes is not better explained by another psychotic disorder
Bipolar I vs Bipolar II?
Bipolar II can never have a full manic episode, they will remain in the hypomanic episodes so it is not impacting their life
Bipolar II DSM criteria
Same as Bipolar I except NO full manic episode and:
criteria needs to be met for one hypomanic episode and major depressive episode; symptoms of depression or the unpredictability cause by frequent alternations between depression and hypomania cause clinical distress
*also cannot be explained by drugs or medical issues or other psychotic disorders
Suicidality and Bipolar
High rates of suicidality w/in this disorder 15x greater risk for completed suicide
Mania
Highly productive, creative state of exuberance
treatment; medication
Lithium and SSRI’s
treatment; psychotherapy
group therapy, CBT, family support group, relapse prevention strategies
