List the unit responsibilities for Environmental Health and Safety Programs.
Unit shall have a Safety Officer and Assistant Safety Manager to ensure safety prevention, investigation mishaps, and undertake all safety responsibilities.
Explain the following related to individual safety: - Electrical Safety - Hazardous Waster Operations and Emergency Response - Occupational Medical Survelliance and Evaluation Program - Hearing Conservation - Hazard Communication Standard - Heat Stress - Respiratory Protection
- Electrical Safety - Obey equipment lock-out/tag-out procedures - HAZWOPER -? - OMSEP - Abide to the requirements of the program - Hearing Conservation - Wear PPE at all times, Ensure enrollment in the unit’s medical program, Recieve annual audiograms if enrolled in the unit’s medical surveillance program for noise. - Hazard Communication Standard - Be aware of all chemical agents encountered in the workspace, the hazards posed by those chemicals, possible symptoms of over exposure, and the PPE requirements to minimize exposure, notify supervisor if ever required to use an unknown substance
- Heat Stress - Seek guidance during unecessary risks; ensure you have received the proper training to identify the signs of injury caused by exposure to thermal stress.
- Respitory Protection Program - Notify of required conditions; wear PPE; attend training; report malfunctions and damage to PPE; change cartridges and canisters
- Pre-Mishap Plan - Be familiar with unit plans
- Confined Space Entry - Only enter spaces after the evaluation of the hazard and other safety concerns.
- Jewelry Wear on Boats - No jewelry during underway evolution; brief during pre-underway briefs.
Explain the Mishap System and Identify the threshold anrequired actions for the following MISHAP levels:
- Class A
- Class B
- Class C
- Class D
- Class E
- High potential for injury, damage, or Coast Guard wide implementation
- Class A: Fatality or total disability; MIA personnel; Damage $2M or greater; Boat Damage $300K or greater.
- Class B: Permanent Partial Disability; $500K to $2M is property damage; Boat Damage $200K to $300K.
- Class C: One or more days away from work; $50K to $500K property damage; Boat damage $50K to $200K
- Class D: Injury not requiring missing from work; $5k to $50k property damage; Aviation damage less than $50K
- Class E: Aviation Flight mishaps or Aviation Ground mishaps only; engine mishaps; FOD mishaps; APU
- HIPO: Any mishap that had the potential to become a Class A/B, waranting the spread of Coast Guard-wide implementation.
Discuss the following for Mishap policy procedures.
- Purpose of the Mishap Analysis Assistance and Team (MAAT) and MAAT membership
- MAAT responsibilities after reciept of information regarding the occurrence of a Mishap
- Mishap Analysis Board (MAB) responsibilities and composition
- MAAT members appointed by CG-731; assists with reporting procedures
- Evidence collection and toxicology reports
- MAB identifies deficiencies that if not corrected expose the organization to future occurences of the mishap event for all classes. Composed of the president, Engineering Advisor, Operations Advisor, Technical Advisor, Supplemental Members
