What are Seborrheic Keratoses?

A

They are weird cauliflower spouting looking things,. (Think, they’re the type of skin lesions Seb will get)

Can appear with vascular parts in them too.

Warty “stuck on appearance”.

Can graduall;y increase in number and be benign, but a dramatic sudden increase in number is a sign of a GI adenocarcinoma

What is sign of leser-trelat?

A

Numeous Sebhorroeic Keratoses appearing suddenly, a paraneoplastic symptom, and can underlie a solid organ malig (usually GI adenocarcinoma)

Epidermoid cyst (often wrongly called sebaceous)
Pilar cyst
Steatocystoma
Dermoid cyst
Hidrocystoma
Ganglion cyst

A

Epidermoid cyst (often wrongly called sebaceous) = proliferation of th eepidermal cells in teh epithelium

Pilar cyst = related to the hair folicle /hair (think erector pilae muscle, so pilar = hair)

Steatocystoma - related to the sebaceous gland

Dermoid cyst - haemotomas

Hidrocystoma - looks like a mini balloon (hidro - hydro, often around the eye)

Ganglion cyst - mucosal degenaration in a joint

dermatofibroimas

A

often after trauma/ scarring, it is a mini ball of proliferated fibroblast cells that are stuck to the upper layer of the skin but moveable above the lower fat, so have positive dimple sign. Can be itchy or tender in which case you’re more concerned

What is a lipoma

A

Proliferation of fat cells. Unknown cause. Smooth and rubbery subcutaneous mass.

Want to check for angiolipoma (blood vessels in) or liposarcoma

A

proliferation of blood vessels in the dermis, eg spider naevi eg pregnancy/liver disease usually asymptomatic

A

Pyo - GENI Granuloma

Gardening whizz has pricked their finger, and its formed a red ball ontop of skin.

“Rapidly enlarging red/raw growth, often at a site of trauma.
Bleed easily
Cause is unknown
Occur in up to 5% of pregnancies
Common on head and hands
Removed by curettage & cautery”

Why is UVA and UV B used on inflammatory skin conditions

A

Because it reduces the immune response.

So if you get burnt, then it’s a double whammy, injury to the cells and reduced immune responce

What is Bowen’s disease?

A

squamous carcinoma that is within the epithelium only, looks like red, flakey irregular lesions, oftne found on womens lower legs. Pre malignant skin condition. Full thickness of the epidermis.

What is photodynamic therapy?

A

Can be used in Bowen’s disease. It’s where you apply a certain photoactive chemical to the lesion and then apply the light. Is mainly taken up by the cancer and so causes infglammtion and essentially will end up killing off the cancer cells.

What is Aldara? (Imiquimoid)

A

Its is an immune responce modifier, cna be used in Bowens. Causes significant inflammation and takes 6 weeks, but good alternative when surgery is undesirable.

What is actinic keratoses?

A

ACT In (It) - Act in the sun (no sun in Aberdeen)

SUN damaged areas, rough scaly patches on sun damaged skin. May spontaneously resolve.

Treated with Cryotherapy, Curettage, diclofenac Gel (not as effective) and Imiquimod
