Barbiturates Flashcards
1
Q
What dose of phenobarbitone is toxic?
A
> 8mg/kg
2
Q
Which barbs can only be given IV?
A
Thiopentone + Pentobarbitone
They’re highly lipid soluble hence only IV
3
Q
Which are the longest acting?
A
Phenobarbitone
Less lipid soluble hence wider distribution and longer duration
4
Q
What blood level causes toxicity?
A
> 30mg/L = increasing sedation
80 = coma
5
Q
How long dose coma normally last?
A
Thio = 24-48 hours
Phenobarb = days to weeks!
6
Q
What treatments can be given?
A
- MDAC as it interrupts enterohepatic circulation of barbs
- Haemodialysis
7
Q
What are the indications for haemodialysis?
A
> 100mg/L
Rising of plateauing levels despite MDAC
In ileus (MDAC won’t work)
8
Q
What the clinical signs?
A
Coma
Hypotonia
9
Q
Does MDAC reduce mortality?
A
NO
Not been shown to reduce LOS in ICU or length of coma
