Tetracyclines MOA, A/e, spectrum coverage?
MOA: inhibit protein synthesis preventing growth of peptide chains & is bacteriostatic (DOES NOT KILL!)
A/e: AVOID in children under 8 y/o (teeth discoloration), pregnant women, interacts with digoxin and warfarin in older adults
Spectrum - gram -ve and gram +ve bacteria SPECIFICALLY MYCOPLASMA
What are Tetracycline pharmacokinetics?
Decreased absorption with food
POOR CSF penetration
Have precautions in renal and liver impairment
Macrolides MOA, spectrum, A/e?
”- mycin “ (Erythromycin)
MOA: inhibit protein synthesis & bacteriostatic (bactericidal in high doses)
Spectrum: Gram +ve bacteria (some gram -ve)
A/e: avoid with CCB (verapamil & diltiazem), potential superinfection of the bowel!
What are Macrolide pharmacokinetics?
Absorption - SI (food decreases absorption)
Clindamycin Spectrum coverage?
Most anaerobic bacteria (gram +ve & gram -ve)
MOST gram +ve aerobes
Used for severe group A strep & Abd/pelvic infx
Clindamycin A/e?
C-diff diagnosis – if present d/c and admin vancomycin
Linezolid MOA, spectrum, A/e?
MOA: inhibits protein synth, binds 23S and. blocks formation of initial complex
Spectrum: used for VRE or MRSA, or serotonin syndrome patients
Linezolid Drug interctions
MAO Inhibitors - can lead to HTN crisis
SSRIs such as paroxetine or duloxetine
Mupirocin is used for what?
Can clear nostrils of MRSA, bactericidal
What 5 things are Linezolid APPROVED for?
Infections by VRE (vancomycin resistant abx)
Pneumonia (hospital or community acquired)
Uncomplicated/Complicated skin infx r/t staph aureus,
What BIG adverse effect can Linezolid cause?
MYELOSUPRRESION! This then leads to anemia, leukopenia, and thrombocytopenia
