A

staphylococci and streptococci more than 90% of cases
Staph aureus in black children
S viridans follows dental extraction or oropharyngeal surgery
valves damaged by rheumatic fever

Clinical features

A

mostly 7-10 year old affected African Children

  • fever anaemia and murmurs
  • clubbing, cardiac failure and splenomegaly
  • petechiae
  • pupura, Roth’s spots absent pulses, haematuria, proteinuria
  • osler’s nodes, splinter haemorrhages and Janeway’s lesions

Diagnosis

A

PUO
Patients with rheumatic disease or congenital heart diseases
with fever and anaemia
positive blood cultures

Treatment

A

treat skin sepsis
antibiotic cover during surgery
IVI combination of antibiotics - penicillin and gentamicin for S viridans and cloxacilli and gentamicin or amikacin for S aureus
