What is Atopic eczema
Chronic atopic condition caused by defects in the normal continuity of the skin barrier, leading to inflammation in the skin.
Causes of atopic eczema
Genetic component - tends to run in families
Environmental triggers
Atopy triad
Eczema, Asthma, Hayfever
Atopic eczema presentation
Usually in children with dry, red, itchy, sore patches of skin over the flexor surfaces and on the face and neck. Can get flares of eczema.
Atopic eczema differentials
Psoriasis (less itchy, plaques), allergic contact dermatitis, fungal infection, scabies
Atopic eczema pathophysiology
Skin becomes thin. Defects in skin barrier means irritants, microbes and allergens can enter tiny gaps. Causes an immune response, resulting in inflammation and associated symptoms.
Atopic eczema management
Maintenance and management of flares - Betnovate topical steroid
Use of emollients to create an artificial barrier over skin (e.g. E45, Diprobase, Cetraben)
Use emollient and steroid cream together
Antihistamines to combat itching
Avoid environmental triggers
Topical tacrolimus
Steroid creams from least strong to most strong for eczema
Hydrocortisone Eumovate Betnovate RD (reduced dose (1/4)) Betnovate Dermovate
Atopic eczema complications
Can get infective eczema - treat with ABX
