Occupational Self-Assessment
Self-report measure
Identifies clients problem areas in occupations, competency in occupations, and level of importance assigned to them
Takes 10-20 minutes to administer
The Occupational Performance History Interview II
Semistructured interview
Takes 1 hour to administer
Focuses on occupational adaptation in three areas: occupational identity, occupational competence, and impact of occupational behavior environment
Generates a narratives of life history and allows therapist to compare performance over time
Allows comparison of performance from patient perspective, so might not be best for acute psych patients

(Or can be administered verbally as necessary)
Identifies roles in which the individual has participated in the past, currently participates in, or hopes to participate in the future
Does not measure occupational performance!

Canadian Occupational Performance Measure
Semistructured interview in three areas: self-care, productivity and leisure
Clients score their performance and satisfaction with identified occupational performance challenges
Useful for goal setting, intervention planning and measuring outcomes
Takes 30-40 minutes to administer
Meaningfulness of Activity Scale
Questionnaire (Likert scale)
Asks participants to rate the level of enjoyment, motivational source, perception of competence, and participation in leisure activities.
Barthel Index
Measures a person’s independence in BADL and level of personal care/assistance needed
Direct observation, interview of individual/caregiver, and review of medical records
Contextual Memory Test
Good tool for evaluation of memory and concentration in a person with TBI
Leisure Diagnostic Battery
Measures an individuals leisure experience and motivational/situational issue that influence leisure
Katz Index of ADL
Assesses only BADL, not IADL
Level of independence and type of assistance required
Catherine Bergego Scale (CBS)
Standardized checklist
Detects the presence and degree of unilateral neglect during observation of everyday life situations
(e.g. dressing, washing, eating, communicating, exploratory activities and moving around)
