Euthanasia Appointments-

Appointments can be made at anytime (for 30mins slots, this does NOT mean the owner only has 30mins) . They should be made as a regular appointment 30 minutes in the Avimark calendar.
Make sure to fill out the Final Gift Crematory Form and have the Owner sign it with private or communal cremation checked off.
Try to have the bill settled before the procedure. Once everything is signed, scan the forms and attach under the Euthanasia code.

There are 2 types of consultations Dr.Kosovsky offers; in person and drop off.

Dr. Kosovsky Appointment- In Person Consultation

For in person consultation: (Usually done at 8:30am)
1-First call Dr.Kosovsky and see what day she is available for an IN PERSON consultation and what time.

2-Second call the owner and see if that time and day works for them. IF it does great!

3-Schedule the appointment in the admission slot and make sure to say IN PERSON consult and put it for the time Dr.Kosovsky will be there.
If the time and day does not work ask them what may work and call Dr.Kosovsky and see if she is available at that time. Try NOT to call Dr.Kosovsky too many times, if the owner is unavailable any of those times they have to do a drop off appointment.

Dr. Kosovsky Appointment- Drop off Consultations

1-First call Dr.Kosovsky and see what day she is available for a drop off consultation.
2-Second call the owner and see if that day works for them. IF it does great! Schedule the appointment in the admission slot and make sure to say DROP OFF consult.
ALWAYS confirm with Dr.Kosovsky after you have confirmed with the owner. The appointment is not
scheduled with Dr.Kosovsky UNLESS you confirm with her after speaking with the owners.
All patients for in person or drop off appointments must be fasted, no food or water after midnight the night before.

Question 20

Dr. Kosovsky Sedated X-Rays

Dr. Kosovsky consultations require sedated x-rays and sedated manipulation of the limb. Therefore the patient will be sedated, requiring an empty stomach. The patient should have recent bloodwork, within 2 months, or there will need an Abaxis done to proceed with anesthesia.

Dr. Kosovsky -Estimates

Make sure an estimate is done at least a week prior to their appointment: It should include sedation, x- rays and Dr.Kosovsky consultation. If it is a drop off then there should also be a charge for day hospitalization for testing. Have the doctor who suggested the consult do the estimate. Blood work should have been done within the last two months, otherwise we can do an in-house Abaxis prior to sedation. Confirm this with the doctor prior to the patient coming in.
Also the doctor should fill out the consultation form, this is found under the client “FORMS” in Avimark under the description Dr.Kosovsky consult forms. It is saved as an attachment.
