Aortic Dissection
definition
intimal tear with blood flowing into tunica media
Aortic dissection can be fatal in at least these three ways
- hypovolemic shock
- retrograde bleeding into pericardial sac
–> cardiac tamponade
- cardiac/end organ ischemia from obstruction
Where do aortic dissections occur?
proximal 10 cm of aorta
(high stress region)
Predispositions for aortic dissection
connective tissue disorders
HTN
Presentation of aortic dissection
sharp, tearing chest pain that radiates to back
Which disease can classically cause a thoracic aneurysm?
tertiary syphillis
endarteritis of vasa vasorum -> luminal narrowing, decreased blood flow, and atrophy of blood vessel wall -> dilation
Major/common complication of thoracic aneurysm
aortic regurgitation
dilation pulls leaflets of aortic valve apart
Strange complications of thoracic aneurysm
compression of mediastinal structures
(airway, esophagus, nerves)
thrombus/embolus
Abdominal aortic aneurysm
definition
balloon-like dilation of abdominal aorta
Where does a AAA normally occur?
below renal arteries
above illiac bifurcation
Underlying pathology of AAA
atherosclerosis
deposits create barrier for diffusion of nutrients from intima to media
wall becomes weak
balloons out
When is a AAA really dangerous?
> 5 cm
Presentation of ruptured AAA
hypotension, pulsatile abdominal mass, flank pain
Unusual complications of AAA
compression of local structures
(ureter)
thrombosis/embolus
