Generalized Anxiety Disorder
A. Anxiety/Worry most of the time for more than six months
B. Inability to manage symptoms
C. 3 of:
- Tension in muscles
- Concentration problems
- Hyperarousal/Irritability

Specific Phobia
A. Persistent and unreasonable fear of a specific object or situation
B. Exposure leads to anxiety response
C. Fear is out of proportion
D. Avoidance of exposure
PTSD
A. Exposure to traumatizing event (direct exposure, direct witnessing, indirect close relationship, indirect aversive details)
B. Intrusive symptom (intrusive memories, nightmares, flashbacks, distress to exposure to reminder, physiologic reactivity to exposure reminder)
C. Avoidance
D. Negative cognition/mood, 2 of:
- Poor recall of event
- Negative cognition
- Distorted blame
- Negative trauma-related emotions
- Diminished Interest
- Alienation
- Flattened affect
E. Hyperarousal, 2 of:
- Irritable/Aggressive
- Reckless
- Hyper-vigilance
- Exaggerated startle response
- Concentration difficulties

OCD
A. Obsessions
1. Persistent, intrusive thoughts that cause anxiety
2. Suppression of the thoughts
or compulsions
1. Repetitive, rigid behaviors in response to an obsession
2. The behaviors reduce anxiety
B. Time-consuming - functional impairment
Panic Attack
A. Abrupt surge of fear/discomfort with 4 of:
- Palpitations
- Sweating
- Trembling
- SOB
- Choking
- Chest pain
- Nausea
- Dizziness
- Derealization/Depersonalization
- Fear of losing control
- Fear of dying
- Paresthesia
- Chills/Hot flushes
Panic Disorder
A. 2 or more panic attacks
B. One attack has been followed by a month of 1 of:
- Worry about more attacks
- Maladaptive behavioral change
Agoraphobia
A. Extreme anxiety with 2 of:
- Public transit
- Open areas
- Closed areas
- Lines or crowds
- Alone outside the house
B. Avoids above situations for fear of being stuck in the event of a panic attack
C. Above situations incite fear/anxiety
D. Above situations are avoided or endured with strong fear
E. Out-of-proportion fear
F. Persistent for 6 months
Social Anxiety Disorder
A. Fear of social situations where the person is exposed to the possible scrutiny of others. Fear that they will act in a way that will be perceived negatively
B. Exposure provokes anxiety
C. Out-of-proportion
D. Exposure avoided or endured with intense anxiety
E. Functional impairment
F. Persistent for 6 months
