Antispasmodics Flashcards
antispasmodics can be divided into 2 main classifications:
antimuscarinics
smooth muscle relaxants
how do antimuscarinics work
reduce intestinal motility and are used for gastro-intestinal smooth muscle spasm
name 4 antimuscarinics used as antispasmodics
atropine sulfate and dicycloverine hydrochloride,
propantheline bromide and hyoscine butylbromide.
which antimuscarinic antispasmodics are less lipid soluble and therefore less likely to cross the BBB so have a lower risk for CNS SE
propantheline bromide and hyoscine butylbromide.
which antimuscarinics are less well absorbed from the GIT
propantheline bromide and hyoscine butylbromide.
…. has a much less marked antimuscarinic action than atropine and may also have some direct action on smooth muscle
dicycloverine HCL
this is advocated as a gastro-intestinal antispasmodic, but is poorly absorbed.
hysoscine butyl bromide
name the 3 direct acting intestinal smooth muscle relaxants
Alverine citrate, mebeverine hydrochloride, and peppermint oil
what would you use to relive abdominal pain or spasm in IBS (3)
Alverine citrate, mebeverine hydrochloride, and peppermint oil
for all antimuscarinincs, what should you advice pt about driving and skilled tasks
Antimuscarinics can affect the performance of skilled tasks (e.g. driving).
dose of hyoscine butylbromide (buscopan) for IBS/GI smooth muscle spasm
10mg TDS
severe interaction with this antipsychotic and all antimuscarinics
caution clozapine
both can cause antimuscarinic effects
both cause constipation - concurrent use may increase risk of developing intestinal obstruction
when can buscopan be sold to the public and for what age
medically confirmed IBS
single dose cannot exceed 20mg
daily dose cannot exceed 80mg
pack cannot contain total of more than 240mg
age: 12 years and over
alverine dose for symptomatic relief of GI disorders characterise day smooth muscle spasm , dysmenorrhoea
60-120mg 1-3x day
can you buy alverine otc
