- colchicine
- allopurinol (uricosuric agent)

colchicine moa
A
- inhibits wbc into body tissue containing urate crystals
- decreases inflammatory reaction to urate crystals in tissue

colchicine indications
A
treatment and prophylaxis of acute gout in adults

colchicine ae
A
- gi: n/v, dyspepsia, abdominal pain
- heme: possible bone marrow supression
- hepatotoxicty
- peripheral neuropathy

colchicine implications
A
- admin with food
- assess for vomit and diarrhea due to dehydration and f&e balance
- monitor for numbness in extremities

colchicine teaching
A
- if have acute flare up, take 1 dose, 2nd dose in 1 hour
- report muscle weakness and severe diarrhea!!!
- low purine diet: avoid etoh, organ/game meats, spinach, peas, anchovies, asparagus

allupurinol moa
A
- inhibits enzyme that changes purine to uric acid thus decreasing uric acid production
- decreases serum and urinary uric acid level
- decreases sx of gout

allupurinol ae
A
- cns: drowsiness, ha, vertigo
- heme: agranulocytosis, bone marrow suppression
- gi: n/v, diarrhea, abdominal pain, indigestion

allopurinol implications
A
- drink lots of fluids
- admin after meals for absorption
- assess cloudy urine
- assess gi effects
- monitor for bleeding, bruising, anemia
- monitor lft, cr clearance

allopurinol education
A
- increased fluid 3-4L a day
- call hcp if decreased uo and cloudy urine
- maintain appointments for uric acid levels, liver enzymes, cr clearance to be drawn
