common s/e of SSRI
nausea weight gain insomnia restlessness agitation dizziness sexual dysfunction: ED, anorgasmia, decreased libido suicidal ideations
onset of SSRI
delayed!
3–4 wks
TCA
tricyclic antidepressants
2nd line tx
blocks reuptake of 5HT and NE
blocks muscarinic and histamine receptors
TCA meds
amitiptyline (Elavil) amoxapine (Asendin) doxepin (Sinequan) imipramine (Tofranil) desipramine (Norpramine) nortriptyline
TCA s/e
anticholinergic: blurred vision, urinary retention, dry mouth, constipation
orthostatic hypotension
can cause cardiac toxicity
MAOI
3rd line tx
increases levels of NE, D2, and 5HT
used for tx resistant depression
MAOI meds
isocarboxazid (Marplan)
phenelzine (Nardil)
tranicypromine (Parnate)
MAOI s/e
lots of interactions with other drugs
must have low tyramine diet- can cause hypertensive crisis
tyramine containing foods
fermented foods, beer, wine, aged cheese, soy sauce, sausages, pickled foods.
