Anti-inflammatory -Steroids Flashcards
Indicate the following for steroids:
1. Indication
2. MOA
- To decrease inflammation
- interfering with the arachoidonic pathway thru inhibition of phospholipase A2 –> decreases inflammatory mediators, capillary permeability, fibroblast and collegen formation –> causes decrease in immune response & wound healing.
indicate the systemic adverse effects of steroids
systemic effects:
* weight gain
* insulin resistance
* diabetes
* HTN
* immune supression
* poor wound healing
* osteoporosis –> most damaging (due to long term use)
indicate the ocular adverse effects of steroids
- PSC catracts
- Increase IOP
- glaucoma
- increased risk of secondary infx
List systemic steroids
- hydrocortisone
- prednisone
- prednisolone
- triamcinolone
- fluticasone – nasal spray
What indicates the strength of an anti-inflammatory med? what systemic steroid med is stronger?
- ## strength depends on how how much glucocorticoid-to-mineralcorticoid activity it has.
List 3 “potent” topical drop steroids
- Prednisolone 1% acetate (Pred Forte)
- Difluprednate (durezol)
- Dexamethasone 0.1%
What is the most potent to least potent of the “potent” steroids?
Difluprednate > dexamethasone > prednisolone acetate
List 2 “soft” topical drop steroids
- Fluoromethalone (FML)
- Loteprednol (lotemax)
Which topical steroids is the safest to Rx? Why?
Loteprednol b/c is ester based
* Pneumonic: LOteprednol –> LOW SE
why are soft steroids safer than potent steroids?
soft steroids are less likely to cause a spike in IOP
What systemic steroids are used as replacment therapy?
- hydrocortisone
- cortisone
opthalmic triamcinolone can be adminster via injection for the following ophtalmic conditions:
- diabetic CSME
- CME associated w/ noninfectious posterior uveitis
- CME secondary to a CRVO
- Irvine-Gass CME
- grave’s orbitopathy
- intermediate & non-resolving posterior uveitis
- chalazion
what are systemic & ocular SE of triamcinolone?
- Elevated IOP
- **depigmentation of the eyelid skin in dark-skinned patients ** (when used for chalazion)
- Endopthalmitis w/ intravitreal injections
Which steroid is a intranasal (topical) corticosteroid?
Fluticasone (flonase)
what are indications for flonase?
- tx of allergic rhinitis
