Anti Arrhythmics Drugs Flashcards
name/class adenosine
Adenocard
Antidysrhythmic
name/class Adenocard
Adenosine
Antidysrhythmic
Description
Adenosine
Adenocard
Adenosine can “chemically cardiovert” PSVT to a normal sinus rhythm. It has a half life of 10 seconds and does not cause hypotension.
Indications
Adenosine
Adenocard
Narrow, Complex Paroxysmal Supraventricular Tachycardia Refactory to Vagal Maneuvers
Contraindications
Adenosine
Adenocard
Atrial Fibrilation, 2nd and 3rd degree heart block, sinus node disease or asthma
Precautions
Adenosine
Adenocard
It may cause transient dysrhythmias especially asytole, COPD
Adult Dosage/Route
Adenosine
Adenocard
6 mg rapidly(over 1-2 sec) IV, then flush line rapidly with saline. If ineffective 12 mg in 1-2 min may be repeated
PEDI Dosage/Route
Adenosine
Adenocard
0.1 mg/kg (over 1-2 sec) IV followed by rapid saline flush, then 0.2 mg/kg in 1-2 min to max 12 mg
Name/class Amiodarone
cordarone, pacerone Anti dysrhythmic (KCL Blocker)
Name/class Cordarone
Amiodarone, Pacerone Anti dysrhythmic (KCL Blocker)
Name/class Pacerone
Amiodarone, Cordarone Anti dysrhythmic (KCL Blocker)
Description
Amiodarone
Cordarone
Pacerone
amiodarone is an antidysrhythmic that prolongs the duration of the action potential and refactory period and relaxes smooth muscles reducing peripheral vascular resistance and increasing coronary blood flow
Indications
Amiodarone
Cordarone
Pacerone
life-threatining ventricular and supraventricular dysrhymias, frequently atrial fibrillation
Contraindications
Amiodarone
Cordarone
Pacerone
Cardiogenic shock, severe sinus bradycardia, or advanced heart block
Adult Dosage/Route
Amiodarone
Cordarone
Pacerone
150 mg over 10 min PT with pulses or 300 mg pulseless IVP
Pedi dosage/route
Amiodarone
Cordarone
Pacerone
5 mg/kg IV/IO, then 15 mg/kg/day
name/class Bretylium
Bretylol
Antidysrhythmic (KCL Blocker)
name/class Bretylol
Bretylium
Antidysrhythmic (KCL Blocker)
Description
Bretylium
Bretylol
Bretylium causes a release of norepinephrine, depresses ventricular fibrillation, and reduces ectopy. Bretylium suppresses ventricular tachydysrhythmias including VFIB with reentry mechanisms.
Indications
Bretylium
Bretylol
Ventricular fibrillation and ventricular tachycardia refactory to lidocane
Contraindictations
Bretylium
Bretylol
None
Dosage/route
Bretylium
Bretylol
5 mg/kg IV, then 10 mg/kg 15-30 min to a max 30 mg/kg
name/class lidocane
xylocaine
Antidysrhythmic (NA+ Blocker)
Name/class xylocaine
lidocane Antidysrhythmic (NA+ Blocker)
Description
lidocane
xylocaine
Lidocane is an antidysrhythmic that suppresses automaticity and raises stimulation threshold of the ventricles. It also causes sedation, anticonvulsant and analgesic effects
Indications
lidocane
xylocaine
pulseless ventricular tachycardia, ventricular fibrilation, ventricular tachycardia (with pulse)
contraindications
lidocane
xylocaine
hypersensitivity to amide-type local anesthetics, supraventricular dysrhythmias, stroke-adams syndrome, 2nd and 3rd degree heart blocks and bradycardia
precautions
lidocane
xylocaine
hepatic or renal impairment, CHF, Hypoxia, Respiratory depression, Hypovolemia, Myasthenia gravis, shock, elderly
adult dosage/route
lidocane
xylocaine
1-1.5 mg/kg IV repeated every 3-5 min up to 3mg/kg consider 1/2 dose for eldely follow conversion with a drip of 2-4 mg/min. 1 gram/250cc
Pedi dosage/route
lidocane
xylocaine
1 mg/kg IV, repeat every 3-5 min up to 3 mg/kg, follow conversion with a drip of 20-50 mcg/kg/min
name/classification
Procainamide
Pronestyl
Anti-arrhythmic (NA+ Blocker)
name/class Pronestyl
Procainamide
Anti-arrhythmic (NA+ Blocker)
Description
Procainamide
Pronestyl
Procainamide prolongs ventricular repolarization, slows conduction and decreases myocardial excitability
Indications
Procainamide
Pronestyl
Ventricular fibrilation and pulseless ventricular tachycardia refactory to lidocaine
Contraindications
Procainamide
Pronestyl
Hypersensitivity to procainamide or procane. Myasthenia gravis, and 2nd or 3rd degree heart block
Precautions
