pathophysiology
anterior chamber becomes infiltrated with neutrophils, lymphocytes, macrophages
usually autoimmune
inflammatory cells in anterior chamber cause….
floaters in patient’s vision
chronic anterior uveitis
more granulomatous (more macrophages)
less severe and longer duration of symptoms (>3mo )
Acute anterior uveitis is assoc with
HLA B27 conditions
- ankylosing spondylitis
- IBD
- reactive arthritis
chronic anterior uveitis is assoc with
sarcoidosis syphilis lyme disease TB herpes
typical presentation
unilateral symptoms starting spontaneously with no Hx trauma
may occur with flare of assoc disease e.g. reactive arthritis
features
painful red eye ciliary flush reduced visual acuity floaters miosis photophobia excess lacrimation hypopyon
ciliary flush
ring of red spreading from cornea outwards
hypopyon
collection WBC in anterior chamber
seen as yellowish fluid in from of lower iris
Mx
same day opthalmology review
steroids
antimuscarinic medications
immunosuppressants: DMARDs, TNF inhibitors
laser therapy, cryotherapy, surgery
