I: Middle of Lateral Surface of Radius

A: Pronates and flexes forearm (at elbow)
Flexor Carpi Radialis
O: Medial Epicondyle of Humerus
I: Base of 2nd Metacarpal Bone
N: Median Nerve (C6, C7)
A: Flexes and Abducts hand at wrist
Flexor Carpi Ulnaris
O: Humeral head: Medial epicondyle of Humerus; Ulnar Head: Olecranon and Posterior Border of Ulna
I: Pisiform Bone, Hook of Hamate Bone, and 5th Metacarpal Bone
N: Ulnar Nerve, C7, C8
A: Flexes and adducts wrist; stabilizes your wrist in neutral
Palmaris Longus
O: Medial Epicondyle of Humerus
I: Distal half of flexor retinaculum and palmar aponeurosis
N: Median Nerve (C7, C8)
A: Felxes hand and tightens palmar aponeurosis
Flexor Digitorum Superficialis
O: Humeroulnar Head: Medial epicondyle of Humerus, Ulnar Collateral Ligament, and Coronoid process of Ulna; Radial Head: Superior half of anterior border of radius
I: Bodies of middle phalanges of medial four digits (branches to the sides)
N: Median Nerve (C7, C8, T1)
A: Flexes middle phalanges at proximal IP joints of medial 4 digits; flexes proximal phalanges at MCP joints and hand
Flexor Digitorum Profundus
O: Proximal 3/4 of medial and anterior surfaces of ulna and interosseous membrane
I: Bases of Distal Phalanges of medial 4 Digits
N: Medial Part–> Digits 4,5 : Ulnar Nerve (C8, T1) ; Lateral Part–> Digits 2,3: Anterior Interosseous Nerve (C8, T1)
A: Flexes distal phalanges at distal IP joints of medial 4 digits, assists with flexion of the hand; medial part flexes DIP of digits 4 and 5; lateral part of digits 2 and 3
Flexor Pollicis Longus
O: Anterior Surface of Radius and adjacent Interosseous Membrane
I: Base of Distal Phalanx of Thumb
N: :Anterior Interosseous Nerve (C8, T1)
A: Flexes Phalanges of Thumb
Pronator Quadratus
O: Distal 1/4 Anterior surface of Ulna
I: Distal 1/4 Anterior Surface of Radius
N: Anterior Interosseous Nerve from Median (C8, T1)
A: Pronates forearm, deep fibers bind radius and Ulna together
