Q

what is the technique for talocrural distraction?

A

in supine, place your hand right over the talus head with the fingers over the talus and the thumbs under the foot and with elbows straight/some bend, lean your body weight back to create jt separation

9
Q

what is the technique for posterior talocrural mobs?

A

in supine, place one hand on the talus and the other on the calcaneous (one hand gripping the heel and the other using the webbed space of the hand on the talus) apply pressure through the talus

may have to PF some

alternatively, you can grab the tibia and fibular with one hand and press down into the talus head with webbed space of the other hand to not touch the calcaneous.

12
Q

what is the technique for mobilization with movement for DF?

A

clinician in the lunging position, patient on a short step/stool

belt around therapist’s butt and the pt’s posterior tibia in line with the pt’s ankle

have the pt weight shift forward into DF while you have the webbed space of your hand (same as post talar mob) on the talus head pushing back

usually done 5 reps at a time

14
Q

what is the technique for anterior talocrural mobs?

A

put force straight down the calcaneous while stabilizing the tibia by wrapping a hand around it

OR in supine with a small wedge with knees bent, grip the head of the talus with your webbed hands and pull into PF

OR in supine with knees bent and foot flat on the table, posteriorly glide the mortis by pushing back just above the talus head

16
Q

what is the technique for subtalar distraction?

A

in prone, cup calcaneous with one hand and stabilize their leg with the other hand to distract the joint

OR

in SL, grip the calcaneous with the webbed space of one hand while the other hand maintains the foot in neutral. Hand stays still and you lean your body weight back

can provide inversion/eversion with these

18
Q

what are the indications for a lateral subtalar glide?

A

to restore subtalar inversion

19
Q

what are the indications for a medial subtalar glide?

A

to restore subtalar eversion

20
Q

what is the technique for lateral subtalar glide?

A

in prone or sidelying with same position as distraction, push the calcaneus laterally

21
Q

what is the technique for medial subtalar glide?

A

in prone or sidelying with the same position as distraction, push the calcaneus medially with the thumb

22
Q

what are the indications for midtarsal mobilizations

A

to restore midtarsal motion (mostly DF, PF)

23
Q

what is the technique for midtarsal mobilization of the navicular on the talus?

A

in prone, with a mobilization wedge or folded towel, rest the foot on the wedge/towel.

stabilize the talus and mobilize the navicular

pincer grasp on navicular

dorsal glide to restore DF, plantar glide to restore PF

24
Q

what is the technique for midtarsal mobilization of the cuboid on the calcaneus?

A

in prone, with mobilization wedge or folded towel, rest the foot on the wedge/towel

with the thumb on cuboid, glide dorsally to restore DF, glide plantarlly to restore PF

25
Q

what is the technique for TMT mobilization?

A

hold the tarsal bone (navicular, cuboid, cuneiforms) stable with one hand and glide the corresponding metatarsal dorsally to restore DF or plantarlly to restore PF

can add distraction to any

26
Q

what are the indications for TMT mobilization?

A

to restore PF/DF

27
Q

what are the indications for metatarsal sweeps?

A

to improve all midfoot and forefoot motions

28
Q

what is the technique for dorsal fulcrum metatarsal sweeps?

A

in supine, place the thumbs in the center of the plantar foot, fingers on top

spread the metatarsals out, flattening the foot by pushing with the fingers dorsally and pulling with the thumbs plantarlly

29
Q

what is the technique for plantar fulcrum metatarsal sweeps?

A

in supine, place the thumbs in the center of the plantar foot, finger on top

pull on top of the foot and push through the plantar surface with the thumbs to “fold” the foot vertically

30
Q

what are some conditions that may indicate need for digit mobilizations?

A

HAV, plantar fasciitis, hallux limitus

31
Q

what is the technique for MTP distraction?

A

stabilize metatarsal, grasp the phalange of the toe and do long axis distraction

32
Q

what are the indications for MTP distraction?

A

to restore MTP motion

33
Q

what are the indications for MTP glides?

A

to improve MTP flexion
(plantar) & extension (dorsal) motions

34
Q

what is the technique for MTP glides?

A

stabilize the metatarsal with one hand and mobilize the proximally phalange with the other

to restore toe extension: glide proximal phalanx dorsally

to restore toe flexion: glide proximal phalanx plantarly

35
Q

what is the MTP glides to restore toe extension?

A

dorsal glide of the proximal phalanx

36
Q

what is the MTP glides to restore toe flexion?

A

dorsal glide of the distal phalanx

42
Q

what is the technique for IP glides?

A

with one hand, stabilize the proximal phalanx and with the other glide the distal phalanx dorsally to restore extension or plantarlly to restore flexion

43
Q

what are the indications for cuboid whip?

A

to improve DF on the cuboid

44
Q

what is the technique for cuboid whip?

A

in prone, with the knee in some flexion and thumb over thumb on the cuboid or navicular and fingers wrapped around the dorsum of the foot

apply force through the thumbs at the cuboid or navicular and maintain the force as you extend the pt’s knee and plantarflex the ankle toward end range

once tissue resistance is enough, high velocity, low amplitude thrust is applied through the thumb contacts as the foot is brought through an elliptical arc of motion that is produced by ulnar deviation of your wrists
