ankle 4 - test and measures Flashcards
what is the FAAM
a self-report outcome instrument developed to assess physical function for individuals with foot and ankle related impairments
what is the onset like for sprains and fractures
sudden traumatic onset
high velocity of ankle displacement
what is the onset like for overuse injuries
slow gradual onset
associated with rep activity
functional limitation for sparin and fractures
WB
gait
and possiblity of lateral involvement
what are the funtional limitations for overuse syndromes
generally limited after a certain amount of activity
what is a order of an ankle examination
ROM
strength
palpation
special tests
gait
what can we look at statically when observing the ankle
foot - arch, forefoot, rearfoot, pronation, supination
tibia - IR, ER, varus or valgus
femur - IR or ER
pelvis - asym landmarks
what can we notice with someones gait
pronation
supination
step length
heel strike
DF/PF
dynamic knee valgus
femoral motion
pelvic control
pronation - flexible or stiff
flexible
supination - flex or stiff
stiff
seen during toe off
what is the anterior drawer test for
anterior stability
lateral or medial ligament testing
what is the talar tilt test for
medial or lateral ligament
what is the ER test for
distal tib fib joint
what is the impingement test for
sinus tarsi sundrome
what is the thompson test for
achilles tendon rupture
what is the compression test for
high ankle sprain
what is the procedure for the anterior drawer test
pt is sitting or supine
stablize the distal leg anteriorly
grasp the distal foot (calacaneus) in the OPP of the ankle and mobilze forawrd
what is the anterior draw test for
testing for ligamentous laxity
which ligament is most tested with the anterior drawer test
Anterior Talo-Fibular Ligament
what is a positive ant drawer test
the talus sublexes or translates anteriorly
what impairment are used to test with the anterior drawer
eversion sprain
inversion sprain
what is a test for the lateral lig of the ankle
inversion sprain
the talar tilt test - also used for eversion sprains
what is the procedure for the talar tilt test
pt is sitting or supine
distal leg is stablized and the ankle is inverted - quickly
what is a positive talar tilt test
pain and laxity
what is the grading of the laxity seen in the talar tilt test
0 - no laxity
3- lot of laxity
what is the impigment test for
sinus tarsi syndrome
what is the procedure for sinus tarsi syndrome
hands- calcaneus in one hand, forefoot in the other hand
bring the foot into PF
place thumb into the sinus tarsi
maintain this pressure and bring the foot into PF and Df
what is a positive impigment test
pain provoked from pressure of the thumb
greater in DF the PF
what is an another name for syndesmotic injuries
high ankle sprain
what is a the test for high ankle sprains
external rotation test
compression test
what is the procedure for the ER test
pt seated or supine
stabilize the leg
PT maintains DF and ER of the foot
what is a postive ER test
pain in the area of the syndesomis - ant or post
over the interosseus
what is the compression test procedure
compress and release the tib and fib together until mid way up the calf
do we start low or high for the compression test
low
high - provoke more seperation and therefore more pain
what is the test for an achilles injury
the calf squeeze test/thompson test
what is the purpose of the thompson/calf squeeze test
to detect a achilles rupture
what is the procedure for the calf squeeze test
the patient is prone and gently squeeze the calf
what are we looking for with the calf squeeze test, what is a positive test
normal - the foot PF with the squeeze
positive: min PF relative to the other side
how do we get to subtalar neutral
the position where the subtalar, talonvicular, and the calcanealcuboid are congruous
centering the navicular on the talus
the dome are neutral
for achilles tendionpathy what do we palpate
mid portion
intertion point
lateral and deep to the tendon to get to the fat pad
what are we looking for with palpation and achilles tendionpathy
recreation of patient pain at the midportion or the insertion of the tendon
what is the royal london hospital test procedure
pt prone with the foot off of the table
Pinch along the tendon to find most palpable location
Release pressure on tendon and have pt actively dorsiflex
Pinch same location with foot in dorsiflexion
what is a positive test for the royal london
Reduced pain with foot dorsiflexed
what is the procedure for arc sign
prone with the foot off of the table
lightly pinch the tendon looking for any nodules in the tendon - bumps
have the patient activily PF and DF
what is a postive test for arc sign
Nodule moves out from between fingers
