Aneurysm and Dissection Flashcards

1
Q

Types of aneurysms

A

True: involves all 3 layers of the vessel wall; saccular or fusiform

False: breaking of the blood wall and formation of a hematoma

Q

Aneurysm pathogenesis

A

Inadequate or abnormal collagen syntheses; Marfans (cystic medial necrosis), Ehlers-Danlos

Excessive collagen degradation - excess MMPs, low TIMPs; smokers

Q

Aneurysm vessel wall pathogenesis

A

Intimal thickening - due to atherosclerosis; causes ischemia of inner media

Systemic HTN - narrows vasa vasorum; ischemia of outer media layer

A
  • Usually ruptures outward

- Double-barreled lumen = inward rupture

17
Q

Cystic medial degeneration is seen in who?

A

Marfan’s

18
Q

Cystic medial degeneration causes what?

A
  • Dissection

- Aneurysm

19
Q

Cystic medial degeneration histology

A

Blue mucinous ground substance that disrupts elastic fibers

20
Q

Aortic dissection; complications

A

Hemopericardium

21
Q

Vasculitis and pathogenesis

A

Immune-mediated

Infection - syphilis, aspergillus (lung)

Unknown - giant cell, takayasu, polyarteritis nodosa

22
Q

Most common form of systemic vasculitis in adults

A

Giant cell arteritis

23
Q

24
Q

Polyarteritis Nodosa; effected organs/arteries

A

Renal and visceral

Lungs are spared

25
Q

Polyarteritis nodosa; special feature

A

All stages co-exist; lesions are not at the same stage
