Necrotising granulomatous inflammation + small vessel vasculitis effecting the respiratory tract and renal systems especially

What are the LRT features of Wegener’s Granulomatosis/Granulomatosis with polyangiitis?
Cough
Haemoptysis
Pleuritis
What are the renal features of Wegener’s Granulomatosis/Granulomatosis with polyangiitis?
Rapidly progressing glomerulonephritis
Haematuria
Proteinuria
What are the dermatolgical and optic features of Wegener’s Granulomatosis/Granulomatosis with polyangiitis?
Palpable purpura
Conjunctivitis
Uveitis
Keratitis (Inflammation of cornea of eye)
What investigations can be done for a patient with suspected Wegener’s Granulomatosis/Granulomatosis with polyangiitis?
ANCA +ive
Dipstick - Haematuria, Proteinuria
CXR - Bilateral nodular and cavity infiltrates
What is Churg-Strauss syndrome/Eosinophilic Granulomatosis with Polyangiitis (EGPA)?
Granulomatous small vessel vasculitis with rapidly progressing glomerulonephritis, GIT bleeding and palpable purpura
What are some of the symptoms of Churg-Strauss syndrome/Eosinophilic Granulomatosis with Polyangiitis (EGPA)?
Severe asthma Sinusitis Tiredness Wt. loss Anaemia Rashes Joint pain
What investigations can be done for Churg-Strauss syndrome/Eosinophilic Granulomatosis with Polyangiitis (EGPA)?
Very high WCC due to Eosionphillia
ANCA +ive
Give Prednisolone and Cyclophosphamide
What is Microscopic polyangiitis?
Inflammation of small blood vessels
Lungs and Kidney commonly effected causing SOB and renal failure
What are the symptoms of Microscopic polyangiitis?
Tiredness Loss of appetite Joint/Muscle aches SOB, Haemopysis Palpable purpura GIT bleeding/pain
What is the management of Microscopic polyangitis?
Cyclophosphamide
Rituximab
Prednisolone
Will be ANCA +ive
