Anaphylaxis Flashcards
anaphylaxis
severe, life threatening, systemic or generalised hypersensitivty reaction
3 things for anaphylaxis
rapid onset + progression of symptoms
life threatening A/B or C
skin or mucosal changes
flushing
hives/ uticaria
angioodema
supported by exposure to known allergen
A
any obstruction
stridor - upper airway obsturction high pitched
jaw thrust
non supported .:. oropharyngeal airway to support
NOW 999 Ambulance as pt unsupported
NOW deliver o2 15L/min bag valve mask
fill bag up
B
breathing rate
20bpm is normal
wheezing
C
pulse - rate upper end of rate = 100bpm (60-100) volume - weak - reduced BP character - regular colour - pale temperature - clammy
clammy + pale = shock
D
AVPU alert voice pressure - trapezius squeeze and supraorbital notch pressure Unresponsive
E
exposure skin or mucosal changes flushing hives rash angiooedema
differential diagnosis
anaphylaxis panic attack asthma attack faint septic shock - if low bp and hives
adrenaline effects
affects sympathetic nervous system
alpha receptor agonist
- - vasoconstrictor
- reduces oedema
beta receptor
- dilating bronchioles
- increases contractility
- suppressing histamine release
post adrenaline what do you do
monitor
pulse, bp, ecg, pulse oximeter
dose of adrenaline
0.5 milligrams = 500 micrograms
1/1000 adrenaline
administer half of 1ml ampule
IM injection
mid thigh
anterolateral of mid 1/3
pull skin taut, inserting needle, aspirating, inject dose, remove needle halfway, release skin, remove needle fully.
main reasons for unconsciousness
vasovagal faint
hypoglycaemic
