Aminoglycosides Flashcards
Aminoglycosides (5 drugs)
Gentamycin Neomycin Streptomycin Amikacin Tobramycin
Aminoglycosides MOA
Bind 30S and interfere with initiation of protein synthesis. Misreading of the genetic code, wrong amino acids inserted. Bacteriocidal
Aminoglycosides administration (special adm for specific drugs)
Parentally (systemic infection) - poorly absorbed from gut Oral for GI treatment Neomycin: topical Streptomycin: IM Amikacin: IV Intrathecal: meningitis
Aminoglycosides: do they ever cross BBB?
No, not even when inflamed
Aminoglycosides - Special considerations with pt with other disease
Reduce dose in renal impairment
Streptomycin - indications
Plague, tularemia, drug-resistant tuberculosis
Tobramycin, gentamicin, amikacin - indications
Infective endocarditis, UTIs, enterobacteriae
Gentamycin + penicillin - indications
Endocarditis (serious enterococcal, stapylococcal, viridans streptococcal infections)
Aminoglycosides - Adverse effects
Drug-induced renal failure (acute tubular necrosis, glomerular toxicity)
Ototoxicity (vestibular & cochlear)
Aminoglycosides most often associated with vestibular toxicity (2 drugs)
Gentamicin, Streptomycin
Aminoglycosides most often associated with cochlear toxicity (1 drug)
Amikacin
Most nephrotoxic aminoglycoside
Neomycin
Neomycin adverse effects
Most nephrotoxic drug
Hypersensitivity (long-term)
