Mallampati I
soft palate, uvula, anterior/posterior tonsillar pillars visible
Mallampati II
tonsillar pillars hidden by tongue
Mallamati III
only the base of uvula can be seen
Mallampati IV
uvula cannot be seen
BURP
backward, upward, rightward pressure
Gold standard of placement of confirmation
2-3 cm or 1’’ above carina, around T2,T3
Failed Airway Algorithm
1) secured airway algorithm
2) 3 attempts, unsuccessful
3) ventilate the patient with BVM, simple airway, blind airway
4) unable to ventilate/oxygenate SaO2 >90%
5) cricothyroidotomy indicated
7 P’s of RSI
preparation preoxygenation pretreatment -LOAD ---lidocaine opiates, atropine for infants, defasiculation dose paralysis with induction protect and position placement with proof post intubation management
Contraindications for SCh, Anectine, Succs
crush injuries eye injuries narrow angle glaucoma history of malignant hyperthermia burns > 24 hours nervous system DO ie Guillain-Barre, Myasthenia Gravis
Malignant Hyperthermia
defect in the skeletal muscle sarcoplasmix reticulum which causes:
masseter spasm/trismus
sustained tetanic muscle contractions
rapid increase in temperature (as high as 110 degrees)
increased ETCO2
tachycardia/HTN
Mixed Acidosis
