Aggressive Periodontitis Flashcards
Features of Aggressive Periodontitis
Client otherwise healthy
Rapid
Inconsistent with amount of plaque and calculus
Familial aggregation
Localized Aggressive Periodontitis
Between puberty and 20years
At least 1 first molar and the rest incisors
No more than 2 other teeth besides first molar and incisors
Robust serum antibody response
Relatively less inflamation
Generalized Aggressive Periodontitis
Under age of 30 but some older
At least 3 permanent teeth other than incisors and 1st molar
Poor serum antibody response
Intense inflammation
Epidemiology of AP
Primary dentition: prevalence 1-5%
Permant: Below 1% but double for Blacks
A. actinomyces and LAP
Associated with it Facultative anaerobe Viirulence factors: Invade across JE and into CT Leukotoxins that kill innate immune cells LPS Collagenases to degrade collagen Immunosuppressive factors
Aggresive Periodontitis and P. Gingivalis
Major one Anaerobic, gm negative Proteinases LPS Inhibits Il-8 which chemotaxis of neutrophils
Inheritance of LAP
most like gene polymorphism
Dominant inheritance
Host response of LAP
impaired neutrophil function More prostaglandin E2 in GCF Antibody against A.a. is super high Lots of IgG Low levels of Ab against P.g in GAP
Smokers and GAP
smoking is risk factor
Smoking increases extent and severity
Have poorer response to treatment
Treatment
Amoxicilln and Metronidazole
ciprofloxacin and Metronidazole
Maybe surgery
Meticulous therapy
