What are the indications?
A patient with primary adrenal failure who is experiencing signs of adrenal crisis
Conditions of this directive?
Paramedics are presented with a vial of hydrocortisone for the identified patient!!!!
AND: one or more of the following
- age related hypoglycemia
- GI symptoms (vommiting, diarrhea, abdo pain)
- syncope
- temp >= 38 degrees or suspected fever
- altered LOA
- age related tachycardia
- age related hypotension
What are the contraindications?
ax or sensitivity
What is the treatment?
Hydrocortisone - IM or IV
dose- 2mg/kg
max- 100mg
ONE DOSE!!!!!
What is one clinical consideration??
IV admin is only for PCP AIV
What are some examples of underlying issues/stressors that may require someone with adrenal failure to call EMS?
- hypoglycemia
- hypotension
- GI issues
- fractures
Should these pts be transported?
YES! they need additional care and follow up!
When do people with adrenal insufficiency need help?
In cases of STRESS when they can become critically ILL.
