Adolescent medicine Flashcards
1
Q
Refeeding syndrome features
A
Hypophosphatemia Hypokalemia Heart failure Peripheral edema Rhabdomyolysis Seizures Haemolysis
2
Q
Refeeding syndrome pathogenesis
A
Phosphate stores depleted by starvation
When refed, glucose causes insulin release
- triggers phosphate potassium and magnesium uptake by cells - lead to low levels in blood
- insulin also causes metabolic processes making ADP and 2,3DPG using up phosphate
Lack of phosphorylated intermediates cause myocardial dysfunction etc
3
Q
Refeeding syndrome risk factors
A
Weight less than ideal body weight (<70% need to be admitted)
Rate of refeeding
Baseline electrolyte levels
First 2 weeks of nutritional rehab
