What is adenomyosis
Endometrial tissue inside the myometrium (muscle layer of uterus)
Adenomyosis presentation
Middle aged woman. Dysmenorrhoea (painful periods), menorrhagia, dyspareunia (painful sex).
May also present with with infertility.
Adenomyosis differentials
Uterine myoma PID Endometrial carcinoma Endometrial polyps Fibroids Endometriosis
Adenomyosis investigations
Enlarged and tender uterus on examination.
Transvaginal ultrasound of pelvis first line.
If not suitable, MRI or transabdominal ultrasound.
Histological examination of uterus after hysterectomy
Adenomyosis management
Similar treatment to menorrhagia: tranexamic acid/mefenamic acid or Mirena coil, COCP
Specialist: GnRH analogue to induce menopause like state, hysterectomy
Adenomyosis complications
Infertility, Miscarriage, Need for caesarean section, postpartum haemorrhage
