ADDENDUM 7/CORRECTIVE LENSES

ANY member looking to receive corrective lenses..

complete SPECTACLE KIT/CORRECTIVE LENS REQUEST FORM (MSU-1)
-members may wear contact lenses with SCBA/APR/PAPR/MASA MILLENIUM

• submit corrective lens prescription that LESS THAN 1 YEAR OLD and contains their PUPILLARY DISTANCE measurement
• members may submit bi-focals lens prescription

-member must get examined on own time

FAX completed MSU -1 and COPY OF CORRECTIVE LENS PRESCRIPTION directly to DEPT. AUTHORIZED VENDOR AND MSU

-vendor will ship to MSU

MSU WILL DELIVER TO UNIT

• upon receipt, member shall immediately insert kit into face piece and make sure it does not interfere with use
• member should arrange to return OLD KIT TO MSU
• if a member has any issues with kit (damaged or replaced ) CALL MSU AND SEND DAMAGED KIT AND PROPER FORM TO MSU
• if kit is stolen/LOST…LOST PROPERTY REPORT OR THEFT/REPORT
