Acute Rhinosinusitis Flashcards
4 paranasal sinuses
frontal, maxillary, ethmoid, sphenoid
what are each of the paranasal sinuses lined with?
pseudostratified ciliated columnar epithelium and goblet cells
initial treatment for acute bacterial rhinosinususitis?
watchful waiting
How long does Acute rhinosinusitis (ARS) last?
<4 weeks
How long does Subacute rhinosinusitis last?
4-12 weeks
How long does chronic rhinosinusitis last?
> 12 weeks
how many episodes of ARS do you need to be considered recurrent acute rhinosinusitis
4 or more episodes of ARS per year
How long before an exposed person develops Acute viral rhinosinusitis (AVRS) and how long can it last
day 1 and it will last <10 days
most common bacteria that cause Acute bacterial rhinosinusitis?
Streptococcus pneumonia, Haemophilus influenzae, moraxella catarrhalis
How long does Acute bacterial rhinosinusitis (ABRS) usually last?
> 10 days
difference between Acute viral rhinosinusitis (AVRS) and Acute bacterial rhinosinusitis (ABRS) ?
symptoms come concurrently with bacterial infection and tend to last longer than 10 days. A viral infection usually lasts less than 10 days and symptoms appear at different times
antibiotics for Acute bacterial rhinosinusitis (ABRS) (if absolutely needed) for patients without risk factors for resistance
Augmentin for 10-14 days
antibiotics for Acute bacterial rhinosinusitis (ABRS) (if absolutely needed) for patients with risk factors for resistance
high does augmentin for 10-14 days
what makes a patient high risk for resistance?
>65 years old hospitalization in the last 5 days antibiotic use in previous month immunocompromised multiple comorbidities severe infection (tem >102)
is patient is allergic to PCN, what antibiotics should you give them for Acute bacterial rhinosinusitis (ABRS) (if absolutely necessary)
Doxycycline for 10-14 days OR
fluoroquinolone. If they can tolerate cephalosporins then give Clindamycin
