Acute Coronary Syndrome Flashcards
leading cause of morbidity and mortality in the US
acute MI
Biomarkers
Myoglobin, Troponin, CK/CKMB, BNP
Is an H&P most helpful in diagnosisng ACS or acute MI in patients who presented with chest pain?
no
cardiac risk factors
diabetes, smoking, dyslipidemia, age >65, lack of physical activity, male, HTN, personal Hx of angina, MI, CVD, TIA, or PVD
An elevated ASCVD score?
7/5%
5 items that best predict CAD as the cause of chest pain
- ) age/sex
- ) Hystory of CAD, CVD
- ) chest pain that is worse with exercise
- ) chest pain not reproducible by palpation
- ) Patient assumption that the pain is of cardiac origin
ASCVD risk calculator is useful for which patients?
patients without pre-existing cardiovascular disease between 40-79
HEART score includes which factors?
History, EKG, Age, Risk Factor, Troponin
Exercise stress test is recommended for who?
people with intermediate probability of ischemic heart disease, they have an interpretable EKG
SPECT Myocardial Perfusion Imaging
higher sensitivity than exercise stress test but more expensive and increased radiation exposure from IV injection
MUGA scan
The test measures how well your heart pumps with every heartbeat using a radioactive tracer
Transesophageal Echocardiogram (TEE)
another way of doing an Echo except this goes down your throat, can diagnose aortic dissection, aneurysm and rupture
Cardiac Magnetic Resonance (CMR), what is it and what does it look at?
it is a high resolution that takes three dimensional images, looks at diseases of the aorta and pericardium, congenital defects and masses
what can an MRI show of the heart?
valve regurgitation, coronary anatomy, ventricular function, overall heart viability (function)
what can a Cardiac Computed Tomography (CCT) scan show?
coronary anomalies, congenital heart disease, RVD, LVD, prosthetic heart valves, anatomic mapping, pre-op cardiac surgery
