What is Acute renal failure
Clinical emergency. Rapid loss of renal function can be life threatening
Aetiology
Usually caused by an event that leads to kidney malfunction
For example dehydration, loss of blood or injury
Nursing diagnoses
Excess fluid retention r/t acute kidney damage
Electrolyte imbalance r/t decreased urinary output
Risk of dehydration r/t increasing compensatory mechanism
Inertventions
Encourage food and fluids
Restore any fluid loss with IVF therapy if required
Regular vital signs
Assessments
Coldspa to access pain in bladder on urination
Braden scale
Peripheral vascular
Rational for:
Encourage food an fluids
Restore any fluid loss with IVF therapy if required
Regular vital signs
Keep patient hydrated and maintain nutrition to give body energy to repair damage
Keep pt hydrated
Manage and recognise any signs or changes
