Abnormal Heart Sounds Flashcards
What creates the S1 heart sound?
Closure of the TV and MV
What creates the S2 heart sound?
Closure of the AV and PV
The pneumonic Kent-Tuck-Y is associated with which heart sound?
S3 (S1-S2-S3)
What is the S3 heart sound associated with in adults?
Ventricular failure (ventricular gallop)
The pneumonic Ten-Nes-See is associated with which heart sound?
S4 (S4-S1-S2)
What is the s4 heart sound associated with?
Forceful atrial contraction against a stiffened, low compliant ventricle (atrial gallop)
What are the defects that create a systolic murmur?
MR (MVP)/TR
AS/PS
VSD
AP shunts
What are the defects that create a diastolic murmur?
AR/PR
MS/TS
Atrial myxoma
What are the Sx’s of MR/MVP?
Fatigue, DOE, Orthopnea
What are the murmur characteristics of MR/MVP?
Prominent at apex, radiates to L axilla
What are the Sx’s of TR?
Prominent V wave in JVP
What are the murmur characteristics of TR?
Blowing murmur increasing with inspiration (Carvallo’s sign)
What is TR associated with?
Pulmonary HTN
Inferior MI/RV infarct
What are the Sx’s of AS?
DOE, angina, syncope, heart failure, 60’s yo, parvis, tardis
What are the murmur characteristics of AS?
Radiates into suprasternal notch/carotid as., Gallavardin phenomenon (radiates to apex to mimic MR)
What are Parvis and Tardis?
Parvis = weak peripheral pulses Tardis = delayed upstroke of carotid as.
What are the Sx’s of PS?
Angina, syncope
What are the murmur characteristics of PS?
Radiates to L shoulder and increases with inspiration (Carvallo’s sign)
What is PS associated with?
TOF, TGA
What are the characteristics of TOF?
PS, VSD, RVH, overriding aorta
What is PR associated with?
Pulmonary HTN
What are the murmur characteristics of PR?
Blowing murmur called “Graham Steell” murmur
What are the Sx’s of MS?
DOE, orthopnea, cough, Ortner syndrome
What are the PE findings of MS?
Blue facies, murmur after S2
What is Ortner syndrome?
Hoarseness due to compression of the LRLN by L atrium
What are the Sx’s of TS?
Prominent A wave in JVP, ascites, hepatomegaly
What are the murmur characteristics of TS?
Increases with inspiration (Carvallo’s sign) and decreases with expiration and valsalva
What are the PE findings of AR?
Wide pulse pressure De Musset sign Corrigan's pulse Quincke's pulse Traube's sign Durozrey's sign Hill's sign Bisferious pulse
What is a Wide pulse pressure?
Larger difference between SBP and DBP
What is De Musset sign?
Head will bob during diastole
What is Corrigan’s pulse?
Rapid upstroke of pulse
What is Quincke’s pulse?
Alternating blanching and flushing of skin
What is Traube’s sign?
Pistol shot sound over artery
What is Durozrey’s sign?
To and fro murmur over femoral a.
What is Hill’s sign?
BP higher in LE than UE
What is a Bisferious pulse?
Pulse with double peak
What is an Austin Flint murmur?
If AR has strong enough flow to hit mitral valve, the MV can’t open and MS murmur is mimicked
What type of murmur is an AR?
Diastolic decrescendo murmur
