Define AA amyloidosis
occurs as a reaction to another illness such as chronic inflammatory dz or infection which causes the liver to produce a protein called serum amyloid A protein (saa) IN HIGH LEVELS
Inflamation –> releases inflammatory cytokines –> hepatic production of SAA protiens –> inadequate breakdown or excess of accumulation –> AA amalyoid accumulation
When inflamation goes on what happens with SAA?
Small amount of protein called AA protein will seperate from SAA and deposit in tissues as AA amyloid
What is AA
It is a secondary dz, IF YOU find it first you want to figure out what is causing it.
-it is mis folded, something with beta sheets
Normal Glomeruli have
evenly dispersed nuclei (purple dots)
Abnromal glomeruli
does not have purple dots
First Sx of AA AMYLODISIS
PEDEMA
Renal biopsy should be done
pedema`, caused by loss of protein in urine which makes fluid leak out of blood vessels and accumulate into tissues
Other Sx include
increase protein in urine (foamy pee)
Further testing for amyloid
red congo stain, or EM
red congo stain
will show an apple green bifrengence if amyloid is present. Preform this after our pt biopsy is positive.
Red congo stain, EM and bipopsy will_______. What do you do next to confrim.
-will confirm renal amyloid
you have to apply an extra stain targting particular amyloid proteins
brown = positive
