A.33.Antiepileptics used in abscence seizures. "Broad spectrum" Antiepileptic drugs. status epilepticus Flashcards
what is valproic acid (valproate)?
broad- spectrum anticonvulsants
what is the mechanism of valproate?
- inhibits T-type Ca²⁺ channels (in thalamic neurons)
- inhibits Na⁺ channels
- ↑ the inhibitory effects of GABA
what are the indications for Valproate?
JJ-Joly Jocker (good for all types of seizures)
including absence seizures
bipolar disorders
migraine prophylaxis
side effects of valproate
"VALPPROaTTE" Vomiting Alopecia Liver damage (hepatotoxic) Pancreatitis P-450 inhibition Rash Obesity (weight gain) Thrombocytopenia Teratogenesis (neural tube defects) Epigastric pain
what antiepileptics are good in pregnancy?
Lamotrigine
Levetiracetam
*according to the internet
what is Lamotrigine?
glutamate antagonist (AMPA receptor)
what does Lamotrigine do?
inhibit Na⁺ and Ca²⁺ channels
what does Levetiracetam do?
interferes with synaptic release of glutamate (SV2A receptor blockage)
what are the indications for lamotrigine and levetiracetam?
broad spectrum
partial seizures
generalized tonic clonic seizures
lamotrigine–> absence seizures as well
side effects of lamotrigine
“SHD”
SJS
Hepatotoxicity
Dizziness
side effects of levetiracetam
depression
nervousness
what does Ethosuximide do?
inhibits T-type Ca²⁺ channels
when is Ethosuximide given?
absence seizures (narrow spectrum) sux--> sucks to have silent (absence) seizures
what is the T1/2 of Ethosuximide?
up to 60 hours
side effects of Ethosuximide
“EFGHIJ”
Ethosuximide causes Fatigue, Gi distress, Itching and sJs.
