A.26. Non benzodiazepine anxiolytics and non-benzodiazepine hypnotics. Pharmacotherapy of anxiety disorders. Flashcards
mechanism of action of Barbiturates
- prolong GABA activity
- ↑ duration of Cl⁻ channel opening–> hyperpolarization
- at high doses–> GABA mimetic activity
- at high doses–> irreversible cellular damage
what is Thiopental?
ultra-short acting barbiturate
what kind of CNS curve does barbiturates have?
linear
what kind of CNS curve does benzodiazepines have?
plateau
what are Secobarbital and Pentobarbital?
short-acting- barbiturates
what is phenobarbital?
long-acting barbiturate
how is phenobarbital excreted from the body?
unchanged in the urine
what is phenobarbital used for?
seizures (not 1st line agent)
what is thiopental used for?
anesthesia (IV)
what are short-acting barbiturates (secobarbital) used for?
sleep disorders, insomnia
sedation
adverse effects of barbiturates
CNS depression (additive effect)
in what case are barbiturates contraindicated?
patients with porphyria
*they induce excessive heme synthesis
barbiturates are potent inducers of…?
CYP450 enzymes
*enhances metabolism of oral contraceptives, warfarin, statin
list the non-BZ’s sedative-hypnotic drugs
Zaleplon Zolpidem Buspirone Melatonin Ramelteon
what is the indication for Buspirone?
generalized anxiety disorders
“I get anxious if the bus doesn’t arrive at one, so I take buspirone”
what are the side effects of buspirone
GI distress
Tachycardia
Paraesthesia (abnormal sensation of the skin)
what are the non-BZ’s given for sleep disorders (sleep-onset insomnia)?
Zaleplon
Zolpidem
“these ZZZ’s put you to sleep”
melatonin (OTC)
Ramelteon
how are non-BZ’s given?
orally
which non-BZ’s have short duration?
buspirone
zaleplon
zolpidem
what type of metabolism do non-BZ’s have?
hepatic P450 metabolism
what are Zaleplon, Zolpidem and Eszopiclone?
GABAᴀ agonists
what is the difference between BZ’s and Zaleplon?
Zaleplon doesn’t produce the anxiolytic, muscle relaxant, or anticonvulsant effect associated with BZ’s
side effects of Zolpidem and Zaleplon
CNS depression (additive with other CNS depressants) dependence liability
how long does the clinical effect of Buspirone take?
1-2 weeks
what is Buspirone?
5-HT₁ᴀ partial agonist
what does Melatonin do?
regulates the sleep-wake cycle
what does Ramelteon do?
activates MT₁ and MT₂ receptors in the suprachiasmatic nucleus
what are the SE of Ramelteon?
Dizziness, fatigue endocrine changes (↓ testosterone, ↑ prolactin)
what is Flumazenil and what does it do?
it’s an antagonist at BZ’s sites on GABAᴀ receptor
how is Flumazenil given?
IV
what T1/2 does flumazenil have?
short
when do we give Flumazenil?
- benzodiazepine overdose
- to reverse anesthesia
- terminating the activity of Zolpidem, zaleplon and eszopiclone
what can’t Flumzenil reverse?
CNS effects caused by barbiturates and alcohols
