Reduce > Hold (metal/non-metal) > Rehabilitate (move, physio, use)
Reduction options
Closed - manipulation or traction (skin or skeletal)
Open - mini-excision or full exposure
Hold options
Closed - plaster or traction (skin or skeletal)
Fixation:
Internal - intramedullary nail/pins, extramedullary plate/screws/pins
External - mono/multiplanar
Rehabilitate options
Use, move, strengthen, weight bear
Consider pain relief
Complications of fractures
General and specific
General - DVT, infection, fat embolus, prolonged immobility (UTI, chest infection)
Specific - neurovasc/muscle/tendon injury, non-union or mal-union Local infection Degenerative change/OA Avascular necrosis Growth disturbance Compartment syndrome
What do you FEEL for in ortho exam?
Pain, effusion, temperature (PET)
What do you LOOK for in ortho exam?
Scars, swellings, deformities, redness

Collaterals
Cruciates
Cartilage (McMurray)
Special tests in Hip exam
Trendelenburg
Thomas’ (fixed flexion deformity)
Tape (leg length discrepancy)
Shoulder exam movements
Movements (active and passive) - ad/abduction, flex/ext, ex/int rotation
I - Impingement (Kennedy-Hawkins tests, arm and elbow 90 degrees, flap up and down + painful arc test)
S - Supraspinatus - empty beer can
I - Infraspinatus - lower external rotation vs resistance (arm at side, elbow bent 90)
T - Teres Minor - upper external rotation vs resistance (arm at 90 abduction, elbow bent 90)
S - Subscapularis - Gerber Lift-Off, place hand on lower back and ask them to push their hand off)
Knee x-ray requests?
Both weight bearing and non-weightbearing
3 views - AP, lateral, sky-line
X-ray signs of osteoarthritis
Loss of joint space
Osteophytes
Subchondral sclerosis
Subchondral cysts
(LOSS)
Shoulder pain differentials
Subacromial impingement
Rotator cuff tear
Frozen shoulder (adhesive capsulitis)
OA, septic arthritis, gout, rheumatoid arthritis

Heberden’s nodes and Bouchard’s nodes
What are they and in what condition are they usually found?
Osteoarthritis
Heberden’s - bony prominence at DIP joints
Bouchard’s - at PIP joints
Lab findings in rheumatoid arthritis
Anti-CCP,
+ve rheumatoid factor
Elevated CRP and ESR
