- Exposure to x-rays of female patients of child-bearing age (Procedures that involve radiation)
An informed consent agreement should include the following information:
- Explanation of procedure and its risks
- Description of benefits and alternatives
- An offer to answer questions about the procedure
- Instructions that the patient may withdraw consent
- A statement informing the patient if the protocol differs from customary procedure
Why do you need to sign a consent form?
When you sign the form, it means
- You received all the relevant information about your procedure from your healthcare provider
- You understand this information
- You used this information to determine whether or not you want the procedure
- You agree, or consent, to get some or all of the treatment options
