8/9/13 Flashcards

1
Q

C1 Esterase Deficency

A

-Causes hereditery angioedema. ACE inhibitors should not be used in patients with hereditery angioedema because it can exacerbate the vasodilation that results in edema.

Q

Infectous Mononucleosis

A
  • Caused by the epstien bar virus
  • Identified by cervical lymphadenopathy, fever, splenomegaly and malaise
  • Epstein Bar virus infects mature B cells throught CD21, complement receptor.
  • The major immune response generated would be CD8 cytotoxic.

Q

Polymyositis

A
  • Disease caused by an overaction of the immune system possibly due to viral infection that results in the activation of CD4 cells, increased MHC 1 expression and eventual activation and inflilatration of CD8 cells into muscle tissue.
  • Signs and symptoms begin with bilateral proximal muscle weakness and fatigue. Elevated creatine kinase and anti-Jo antibodies is also seen.

Q

Polymyalgia Rheumatica

A
  • Presents with proximal weakness similiar to polymyositis and bilateral stiffness.
  • Associated with temporal arteritis and an elevated ESR.

Q

Lambert-Eaton Syndrome

A
  • Paraneoplastic syndrome
  • Muscle weakness that improves with exercise
  • Autoantibodies to Ca channels are the cause.

Q

Myasthenia Gravis

A
  • Muscle weakness that gets worse with exercise and extraocular involvment (ptosis) are the signs.
  • Antibodies directed to the Ach Receptor are the cause.

Q

Hematopoetic Depression in “normal” patients

A

-Epstien Bar virus, that infects B cells through CD 21 can cause a hematopoetic depression and anemia. Not as severe as aplastic crisks in Parvovirus B19.

Q

Injection Skin Reaction Hypersensitivities

A

-Any injection skin reaction like TB test or other test or contact dermatitis are mediated by T cells, that call in macrophages by secretion of IFN-gamma to the site.
