Legionella pneumoniae

  • Pneumonia with high fever dehydration and diharrea. Also hyponatremia
  • Sputum sample does not show bacteria because of difficulty to stain
  • Urine test is diagnsitc

  • Pneumoniae with AFB that are difficult to gram stai. Often infects young adults

A
  • Appears somewhat similiar to MTB
  • Difficult to stain and AFB
  • Lives in marine environment and often contaminates water. Non transmissable from human to human

Post Streptococcal Glomerulonepphritis

A
  • Kidney damage and edema following streptococcal infection (Impetgigo, folliculutis, etc)
  • Type 3 hypersensitivity, lumpy bumpy deposition of Ab that are cross reactive to self on glomerular membrane.

A
  • Hepatitis A is a self limiting disease that does not have a chronic disease state or a carrier state.
  • The disease often presents anicterically (without jaundice) in children

Hemolytic Disease of the Newborn

A
  • Type 2 hypersensitivity
  • Mothers with type O blood often make antibodies to both types A and B blood. These antibodies are IgG and can cause disease on the first pregnancy (different than Rh)
  • Mothers type A or B make IgM Ab which cannot cross the placenta and do not cause hemolytic disease of the newborn in this case. Rh still can.
  • Disease from Rh antigen is more severe than disease from AB antigen.
  • Can be screened by coombs test, serum causing agglutination of RBCs

7 y/o Lack of Thymus with very low serum gammaglobulin with recurrent infections

A
  • SCID is the correct answer. Shows as a lack of B and T cells with nearly absent Ig. Many causes, X linked, bare lymphocyte, Omen, Adenosine deaminase.
  • DiGeiorge is not correct because it is more rare and has a less severe case of immunodeficency with diminshed levels of B and T Cells and decreased Ig. Also there are no other findings charaacteristic (facial anomilies, tetrology of falot, etc)
  • X linked agammaglobunemia -lack btk and inability for B cells to mature
  • Wiskott-Aldrich:X linked, eczema, thrombocytopenia, immunodeficency, problem with immunological synapse
