What is the cause of reactive arthritis?

A

Posturethritis - chlamydia or ureplasma urealyticum

Postenteritisw- shigella, yersinia , salmonella

HLAB27 genetic predisposition

Affects upping men

What are the clinical features of reactive arthritis

A

1-4 weeks of latency ( preceding diarrhea and dysurea and pelvic pain urethritis and prostatitis)

Oligoarthritis - asymmetrical and migratory in the lower extremities

Sacroilitis

Enthesitis

Dactylitis

What are the extrasolar manifestations of reactive arthritis

A

Conjunctivitis or iritis

Dermatology - balanitis circinata
Hypokeritinisation Of palms and soles
Oral ulcers

Diagnosis of reactive arthritis

A

ESR and crp high
HLAB27 positive

Preceding infection for stool and uterine culture

Microscopy and culture of synovial fluid

  • WBC
    Polly morphonuclear leukocyte predominant

What is the treatment for reactive arthritis

A

NSAIDS
Cryotherapy and physiotherapist

If maids are ineffective intestinal or oral glucocorticoids

Chronic cases - methotrexate and sulfasalazine

Usually resolves spontaneously within a year
But high rate of reoccurrence ESP with HLAb27
