Microvascular complications
Small vessel damage; MC T1DM
- diabetic opthalamology & diabetic nephropathy
Macrovascular complications:
Arteriosclerosis dz of large vessels; MC T2DM
—> CAD; MI; stroke: peripheral vascular dz
Microvascular complication are directly related to:
- glycemic control
Macrovascular complication are directly related to:
- the usual RF’s (lipids, HTN) & glycemic control
This enhances the risk for all complications extensively
Cigarette smoking
Major cause of death: T1DM
Proliferative retinopathy and ESRD
Major cause of death: T2DM
MI and CVA
First line tx of dislipidemia in DM
Statins @ moderate to high intensity dose
Simvastatin (zocor)
Atorvastatin (lipitor)
Rosuvastatin (crestor)
Lipoprotein abnormalities in T1DM
Slight elevation LDL and TG
Lipoprotein abnormalities in T2DM
Increase in TG (300-400 mg/dL)
Decrease in HDL (<30 mg/dL)
More atherogenic LDL
Major ASCVD risk factors-
1- fam hx
2- low hdl (<40)
3- HT
4- smoking
DM but no other major risk factors and/or age <40
LDL-C: <100 mg/dL
TG: <150 mg/dL
DM with +1 major ascvd risk factor or pre-existing CVD:
LDL-C: <70 mg/dL
TG: <150 mg/dL
Microalbuminuria lab numbers
30-300 mcg PRO/day
