3.Rotator Cuff Testing & AC Joint Flashcards

DROP ARM - CODMANS - like grabbing slippery fish

ABD 90* & slowly lower arm to neutral Reason: See if ROT cuff tear or supraspinatus tendon rupture Assessment: ABD 90* & slowly lower arm to neutral Positive: Inability to perform slow, controlled ADD of arm to neutral Or severe pain

EMPTY CAN - JOBES

Both arms ABD 90*Thumbs up
90* & slightly anteriorly w thumbs down
Reason:
Supraspinatus Tendinopathy
~~~
Assessment:
Both arms - 90*ABD Hor
pressure downwards
Both arms 90*ABD slightly ant
Apply pressure downwards
5 seconds
Observing for pain or discomfort
~~~
Positive:
Pain w resisted supraspinatus contraction
Clinical Impression:
Pain possibly due - supraspinatus mm or tendon tear or suprascapular nerve neuropathy

HAWKINS-KENNEDY IMPINGEMENT *supraspinatus*

90* FLEX forward, ADD & MED ROT
Look for expression of pain
Reason:
Supraspinatus Tendinopathy
Positive:
Pain @ shoulder w medial ROT
Clinical Impression:
Pain possibly due - supraspinatus Tendinopathy

HAWKINS-KENNEDY IMPINGEMENT - Dumn guy

90* FLEX forward, ADD & MED ROT
Look for expression of pain
Reason:
Supraspinatus Tendinopathy
Positive:
Pain @ shoulder w medial ROT
Clinical Impression:
Pain possibly due - supraspinatus Tendinopathy

NEERS IMPINGEMENT

SCAPTION ( oblique angle between forward FLEX & ABD)
THUMBS DOWN & then UP
Reason:
Supraspinatus Tendinopathy
AC Joint Pathology
Assessment:
SCAPTION with Thumbs down, then up

AC SHEAR - AC Pathology

form an A & C with hands
Reason:
Abnormally large amounts of movement at ACJ w compression
~~~
Assessment:
Find Spine of Scap
Find Clavicle
Interlock fingers and form an A over shoulder
Apply pressure with both hands
~~~
Positive:
Abnormally large amounts - movement - ACJ w compression
Clinical impression:
Abnormally large amounts - movement @ACJ w compression possibly due to AC joint pathology

SERRATUS ANT WEAKNESS aka “PUNCH OUT”

Fist, push up into therapist hand
Reason:
SERRATUS ant mm weakness
Assessment:
Standing,
90* Shoulder FLEX

SPEEDS TEST - STRAIGHT ARM - BICEP TEST

90* Forward ABD, Hand Pronated & then Supinated Reason: Bicipital Tendinopathy Assessment: 90* Forward ABD, Hand Pronated & then Supinated Pressure on arm to test for strength Positive: Pain near bicipital groove NBN - Negative but notable: Weakness w resisted supination - indicates possible rupture of distal bicipital tendon Clinical Impression: Pain due to bicipital tendinosis
