ANTERIOR APPREHENSION - CRANK TEST
Reason: anterior shoulder dislocation. SLOWLY!!!!!!!!
Supine, STOP sign
Stabilize shoulder
90* ABD
90* Flex - elbow
Passively ROT Arm Externally
Positive: Increased apprehension of pain with slow passive external ROT of arm
Look for: Apprehension due to instability of GHJ w ant translation of Humus
POSTERIOR APPREHENSION TEST - STRESS TEST
Forward FLEX & Med ROT
Reason: Posterior instability of GHJ
Assessment: Supine, Stabilize opposite shoulder,
Forward Flex 90*, Slightly HOR ADD, Pressure on Elbow Joint while MED ROT
Look for: Appreh / Pain - due to instability GHJ w post translation of humerus
Positive: Increased appreh/pain w slow passive post translation of humerus
SULCUS SIGN
“ Sulcing” - downwards, inferior
Reason: See if - Inferior Instability of the GHJ or GH laxity
Assessment: Seated Stabilize shoulder Arm 20-50* from body Inf Pressure @ Cubital Fossa Observing Inf Translation of Humeral Head
Look for: Sulcus sign - indent
Positive: Inf Translation of humeral head inferior to acromion
ANTERIOR DRAWER TEST - close a drawer
Pull Humerus Anteriorly
Reason: Assess - Anterior instability of GHJ
Assessment:
Supine, Client totally flex arm,
Take hold of Bicondylar axis of humerus in order to control arm
Palpate for the head of humerus and translate humerus anteriorly
Positive: Abnormal large amount of movement of humeral head anteriorly or patient apprehension of ant translation
Clinical impression: Abnormal large ant translation or apprehension possibly due to ant instability of GHJ
POSTERIOR DRAWER TEST - open a drawer
Push humerus away from you, posteriorly
Reason: Posterior instability - GHJ
Assessment:
Supine, Fully Flex arm, Take hold of Bicondylar Axis of humerus
Humeral Head of Humerus
Push Posteriorly
Positive:
Abnormal large amount - movement - humeral head posteriorly
Patient Apprehension of Post Translation
Clinical Impression:
Abnormally large post translation of humerus or appreh due to post instability of GHJ
