3.9.2 Nuclear Instrumentation

A

2 source range neutron flux monitors shall be operable and 1 source range audible count rate circuit shall be operable.

Mode 6

A. 1 required source range inop
A.1 Suspend core alts and
A.2 Stop water adds to RCS
Both immediately

B. 2 requires source range inop
B.1 Initiate action to restore 1 source range - Immediately

C. Required audible count rate inop
C.1 Initiate action to isolate unborated water sources - immediately

3.9.3 Containment Penetrations

A

The ctmnt penetrations shall be in the following status:
A. Equipment hatch closed and held in place by 4 bolts
B. One door in each airlock capable of being closed
C. Each penetration providing direct access from ctmnt atmosphere to outside atmosphere is either:
1. Closed by man or auto isolation sig, blind flange, or equivalent
2. Capable of bine closed by Operable ctmnt purge supply and exhause system

During movement of irradiated fuel in ctmnt.

A. 1 or more ctmnt penetrations not in required status.
A.1 Suspend fuel moved in ctmnt - Immediately

3.9.4 RHR and Coolant Circulation - High Water Level

A

1 RHR loop shall be operable and in operation

Mode 6 with water level >/=23 ft above top of rx vessel flange

Note: Required RHR loop may be stopped for </=1 every 8 hours provided unborated water not introduced to RCS.

A. RHR loop requirements not met
A.1 Suspend operations that could introduce unborated water to RCS and
A.2 Suspend loading irradiated fuel assemblies in core and
A.3 Initiate action to satisfy RHR loop requirements
All actions immediately

3.9.5 RHR and Coolant Circulation - Low Water Level

A

Two RHR loops shall be operable and 1 in operation

Notes:
1. All RHR can be stopped for </= 15 minutes when switching loops provided:
A. Core outlet temp maintained at least 10F below sat temp.
B. No ops permitted that would reduce boron concentration
C. No draining allowed
2. 1 required RHR loop can be inop for up to 2 hours for surv testing.

Mode 6 with water level <23 ft above top of rx vessel flange

A. Less than required number of RHR loops operable
A.1 Initiate action to restore req RHR OR A.2 Initiate action to establish >/= 23 ft above top of rx vessel flange
Both actions immediately

B. No RHR loops in operation
B.1 Suspend ops that could introduce unborated water to RCS AND
B.2 Initiate action to restore on RHR loop
Both actions immediately

A

Refueling cavity water level shall be maintained >/=23ft above top of rx vessel flange

During fuel moves in ctmnt

A. Refueling cavity water level not within limits
A.1 Suspend movement of irradiated fuel in ctmnt - Immediately
