3.6.1 Containment
OPERABLE
APPLICABILITY: MODES 1, 2, 3, and 4
ACTIONS
A. Restore within 1 hour
B. A not met, be in MODE 3 within 6 hours and MODE 5 within 36 hours
3.6.2 Two containment air locks shall be OPERABLE
APPLICABILITY: MODES 1, 2, 3, and 4
ACTIONS
- NOTES -
1. Entry and exit is permissible to perform repairs on the affected air lock components.
2. Separate Condition entry is allowed for each air lock.
3. Enter applicable Conditions and Required Actions of LCO 3.6.1, “Containment,” when air
lock leakage results in exceeding the overall containment leakage rate acceptance criteria.
A. One or more air lock with one door inop, verify the OPERABLE door is closed within 1 hour, Lock OPERABLE door within 24 hours. Verify OPERABLE door is locked 1x/31 days (Can be admin means if in high rad area)
(NOTE: Entry and exit is permissible for 7 days under administrative controls if both air locks are inoperable.)
B. One or more air lock with interlock broken, verify an OPERABLE door is closed within 1 hour, Lock an OPERABLE door within 24 hours and verify OPERABLE door locked 1x/31days (Can be admin means if in high rad area)
(NOTE: Entry and exit of containment is permissible under the control of a dedicated individual.)
C. One or more air lock inop for reasons other than A or B, IMMEDIATELY initiate action to evaluate overall containment leakage AND verify a door is closed in the affected air lock within 1 hour AND restore air lock to OPERABLE status within 24 hours.
D. Required action and completion time not met, MODE 3 in 6 hours and MODE 5 in 36 hours.
3.6.3 Each containment isolation valve shall be OPERABLE (except CIVs associated with closed systems and for vacuum relief valves)
APPLICABILITY: MODES 1, 2, 3, and 4
ACTIONS
- NOTES -
1. Penetration flow path(s) may be unisolated intermittently under administrative controls.
2. Separate Condition entry is allowed for each penetration flow path.
3. Enter applicable Conditions and Required Actions for systems made inoperable by
containment isolation valves.
4. Enter applicable Conditions and Required Actions of LCO 3.6.1, “Containment,” when
isolation valve leakage results in exceeding the overall containment leakage rate
acceptance criteria.
A. One or more penetrations with one CIV INOP
A.1 Isolate flow path with a manual/check valve, closed automatic (deactivated) valve ,or blind flange within 4 hours AND
A.2 Verify the penetration isolated 1x/31 days if outside containment and prior to going into MODE 4 from 5 if not done within the last 92 days for valves inside containment (may be administrative for high rad)
B. One or more penetrations with 2 CIVs INOP
B.1 Isolate the flow path with at least one closed (deactivated) automatic valve, manual valve or blind flange within 1 hour
C. Required actions not met BE in MODE 3 within 6 hours and MODE 5 within 36 hours.
3.6.4 Containment Pressure
Maintained ≥ -0.2 psig and ≤ +1.0 psig
APPLICABILITY: MODES 1, 2, 3 and 4….MODES 5 and 6 without an open flow path ≥ 6 inches diameter (high pressure limit not applicable in MODES 5 and 6)
ACTIONS
A. Not within limits, restore within 1 hour
B. Action and completion time of A not met (MODES 1-4), be in MODE 3 within 6 hours and MODE 5 within 36 hours
C. Action and completion time of A not met (MODES 5-6), OPEN a containment air flow path ≥ 6 inches diameter within 8 hours.
3.6.5 Containment Air Temp
Maintained ≤ 120°F
APPLICABILITY: MODES 1, 2, 3 and 4….MODES 5 and 6 with both containment equipment hatches and both containment air locks closed
ACTIONS
A. Not within limits, restore within 8 hours
B. Action and completion time of A not met (MODES 1-4), be in MODE 3 within 6 hours and MODE 5 within 36 hours
C. Action and completion time of A not met (MODES 5-6), OPEN containment equipment hatch or air lock within 8 hours.
3.6.6 Passive Containment Cooling System (PCS)
OPERABLE with Water temp ≥ 40°F and ≤ 120°F, Volume of 756,700 gallons.
APPLICABILITY: MODES 1, 2, 3, and 4…MODES 5 and 6 with decay heat > 7 MWt
ACTIONS
A. One PCS flow path INOP, restore within 7 days
B. Two PCS flow paths INOP, restore with 72 hours
C. One or more water storage tank parameters not within limits, restore within 8 hours
D. LCO not met for other reasons, or completion time of actions A/B/C not met, be in MODE 3 within 6 hours and MODE 5 within 84 hours.
E. LCO not met for other reasons, or completion time of actions A/B/C not met (MODES 5), IMMEDIATELY initiate action to establish pressurizer level ≥ 20% with RCS intact and IMMEDIATELY suspend positive reactivity additions.
F. LCO not met for other reasons, or completion time of actions A/B/C not met (MODES 6), IMMEDIATELY initiate action to establish water level ≥ 23ft above the flange and IMMEDIATELY suspend positive reactivity additions
3.6.7 Containment Penetrations
The containment penetrations shall be in the following status:
a. The equipment hatches closed and held in place by four bolts or, if open, can be closed prior to steaming into the containment.
b. One door in each air lock closed or, if open, can be closed prior to steaming into the containment.
c. The containment spare penetrations, if open, can be closed prior to steaming into the containment.
d. Each penetration providing direct access from the containment atmosphere to the outside atmosphere, if open, can be closed by a
manual or automatic isolation valve, blind flange, or equivalent prior to steaming into the containment.
APPLICABILITY: MODES 5 and 6
ACTIONS
A. 1 or more penetration not in required status, restore within 1 hour
B. Required action/completion time of A not met or LCO not met for reasons other than A
B.1.1 If in MODE 5, IMMEDIATELY initiate action to establish ≥ 20% pressurizer level with the RCS intact OR
B.1.2 If in MODE 6, IMMEDIATELY initiate action to establish cavity level ≥23 feet above flange
AND
B.2 IMMEDIATELY suspended positive reactivity additions.
3.6.8 pH Adjustment
pH adjustment baskets shall contain ≥ 26,460 lbs of trisodium phosphate (TSP).
APPLICABILITY: MODES 1 - 4
ACTIONS
A. Weight not within limit, restore within 72 hours.
B. Action A not met, BE in MODE 3 within 6 hours and MODE 5 within 84 hours
3.6.9 Vacuum Relief Valves
Two Vacuum relief check valves and two vacuum relief isolation valves shall be OPERABLE AND
Containment inside to outside differential temp shall be ≤ 90°F.
APPPLICABILITY: MODES 1 - 4….and 5&6 without an open air flow path ≥ 6 inches in diameter
- NOTE -
Vacuum relief valve OPERABILITY for closing is only required in MODES 1, 2, 3, and 4.

ACTIONS
A. One vacuum relief check valve INOP for opening, restore within 72 hours
B. One vacuum isolation valve INOP for opening, restore within 72 hours.
C. One or more vacuum relief check valves INOP for closing or One or more vacuum isolation valves INOP for closing, restore within 7 days.
D. One or more vacuum relief check valve AND isolation valve INOP for closing, RESTORE both vacuum relief check valves OR both vacuum relief isolation valves within 1 hour
E. Containment inside to outside temp > 90°F, Restore to within limit OR reduce containment average temp ≤ 80°F within 8 hours.
F. Required action and completion time of A/B/C/D/E not met in MODES 1-4 OR 2 vacuum relief check or isolation valves INOP for OPENING in MODES 1-4, Be in MODE 3 within 6 hours and MODE 5 within 36 hours.
G. Required action and completion time of A/B/C/D/E not met in MODE 5 or 6 OR 2 vacuum relief check or isolation valves INOP for OPENING in MODE 5 or 6, OPEN containment air flow path ≥ 6 inches in diameter within 8 hours
