3.3.2 - RTS (Source Range Inst)

A

4 RTS SR channels shall be OPERABLE
* Applicability: Mode 2 (with IR flux below P6) and Mode 3, 4 and 5 with PLS capable of rod withdrawal or one or more rods not fully inserted

Actions
A. One INOP in Mode 2, place in bypass or trip within 2 hours
B. 2 INOP in MOde 2, one in trip and one in bypass within 2 hours
C. Required completion time of A or B not met, IMMEDIATELY suspend positive reactivity addition
D. 1 or 2 channels inop in Modes 3,4,or 5, restore 3 of 4 channels to OPERABLE within 48 hours
E. Action D not met, within 1 hour, inset all control rods and make rod control not capable of withdrawal
F. 3 or more channels INOP, IMMEDIATELY open RX trip breakers

3.3.3 - RTS (Intermediate Range Inst)

A

4 IR RTS channels shall be OPERABLE
Applicability: Mode 1 with Power below P10 and Mode 2
Actions:
A. One Channel INOP with power ≥ P-6, place INOP channel in trip or bypass within 2 hours OR reduce power < P6 within 2 hours OR, Increase power to > P10 within 2 hours
B. Two Channel INOP with power ≥ P-6, Same as A but place one in trip and one in bypass
C. One or two channels INOP < P6, restore 3 out of 4 channels to OPERABLE prior to going above P6
D. 3 or more channels INOP, IMMEDIEALY suspend postive reactivity addition AND lower power <P6 within 2 hours and Mode 3 within 7 hours

A

Four divisions of RTS Auto Trip logic OPERABLE

Applicability: Modes 1, 2, (and Modes 3, 4, and 5 with PLS capable of rod withdrawal or not all rods fully inserted)
Actions:
A. One or two divisions INOP in Mode 1 or 2, restore 3 of 4 within 6 hours
B. A not met, or 3 or more INOP in Mode 1 or 2, Mode 3 in 6 hours
C. One or 2 divisions INOP in Mode 3-5, restore 3 of 4 within 48 hours
D. C not met, or 3 or more division INOP in Modes 3-5, Within 1 hour, fully insert all rods and make rod control incapable of rod withdrawal

A

Four Divisions of Rx Trip Breakers and undervoltage and shunt trip mechanisims on in service RTBs shall be OPERABLE

Applicability: Modes 1, 2 (and Modes 3 - 5 with PLS capable of rod withdrawal or one or more rods not fully inserted).
Note: Separate condition entry is allowed for each function
Actions:
A. One or both Functions within one division INOP, open affected breaker within 8 hours.
B. One or both functions within two divisions INOP, restore 1 to operable within 1 hour
C. Completion time of A or B not met in modes 1 or 2, OR 3 divisions INOP, Mode 3 within 6 hours
D. Completion time of A or B not met in Modes 3-5, or 3 or more division INOP in Modes 3-5, Within 6 hours, fully insert all rods and make rod control incapable of rod withdrawal

Actions:
A. One or more startup feedwater lines with one channel INOP, place in trip within 6 hours.
B. Completion time not met, OR 1 or more startup feedwater lines with 2 channels INOP, Mode 3 within 6 hours and Mode 4 with RNS in service within 24 hours.

3.3.16 - ESFAS Actuation Logic - SHUTDOWN

A

Four Divisions shall be OPERABLE with Coincidence Logic (LCL) and ESF actuation (ILP/SRNC/CIMs)

Applicability: Modes 5, 6 and irradiated fuel movement
Note: Separate condition entry is allowed for each function
Note: Only the divisions necessary (A, B, & C) to support MCR Isolation, Air Supply Initation, and Electrical Load De-Energization are required to be OPERABLE while defueled and moving irradiated fuel
Actions:
A. One or more in 1 division INOP, restore within 72 hours
B. While in Mode 5 Time not met in A or One or More functions within two or more divisions INOP, IMMEDIATELY Suspended postive Rx addition, initiate actions to OPEN RCS and establish ≥ 20% pressurizer lvl, and initiate action to isolate the flow path from the DWS to the RCS be at least one closed/deactivated automatic or manual valve
C. While in Mode 6 Time not met in A or One or More functions within two or more divisions INOP, IMMEDIATELY Suspended postive Rx addition, initiate actions to establish water level ≥ 23 feet above the top of the reactor vessel flange.
D. During irradiated fuel movement Time not met in A or One or More functions within two or more divisions INOP, IMMEDIATELY Suspended fuel movement.

3.3.17 - PAM Instrumentation

A

Shall be OPERABLE IAW table
APPLICABILITY: Modes 1 - 3

Notes:
LCO 3.0.4 is not applicable
Separate condition entry is allowed for each Function

Actions:
A. One or more functions with one channel INOP, restore within 30 days
B. Required completion time of A not met, 14 day report
C. One or more functions with 2 channels INOP, Restore 1 within 7 days
D. Required completion time of C not met, Mode 3 6 hours, Mode 4 12 hours

3.3.18 - Remove Shutdown Workstation (RSW)

A

Shall be OPERABLE

Applicability: Modes 1,2,3 and 4 with RCS Tavg ≥ 350°F.
Notes: LCO 3.0.4 is not applicable
Actions:
A. RSW INOP, restore within 30 days
B. Completion time not met, Mode 3 in 6 hours and Mode 4 Tavg<350°F in 12 hours
