involves the systemic and continuous collection of data- sorting, analyzing, and organizing data
documentation and communication of data collected
gathering info concerning the patients individual physiological, sociological, and spiritual needs
Subjective and objective data collection are an intregral part of this process
Types of Nursing assessments
Comprehensive or initial - full health history and physical exam
focused - assessing a specific problem or system. ongoing care to determine status of a specific pre-identified problem
Full bed side - head to toe assessment with focused subjective and assessment
expedited bedside - done in an emergent situation to assess physiologic or psychologic crisis. ex. rapid assessment of A, B, C’s during cardiac arrest or suicide assessment
